These four spheres play a significant role in opportunities for nurses to get politically involve. Government sphere focuses on laws and regulations concerning nursing practice, as well as reimbursements for healthcare, and influencing local, state, and national policies (Nault, 2012). Government can also determine what health services people are eligible for at certain age, and determines who receives what type of health services.

The differences and variation in the ANP scope of practice among the nation, does not allow for others especially legislators, to have a clear view of the ANP nurse and what it can be accomplish it all the restrictions and barriers were eliminated. It will also change the public perception and attitudes towards nurse practitioners. From the research, it is believed by the author, that since the licensing is done at the state level, this is where changes in America should be implemented. APNs need lobbyists to advocate for the changes needed. It appears that in the states where there APNs have restricted practices, it is due to lobbying by the state’s medical board. The legislators should be presented with the facts, APNs provide safe and cost effective care to the patients. Citizens should get involved and write letters to make the legislators aware of the problem.
Lee, regarding about challenges in changes processes, as nurse leaders, we need to think outside our own silos, research how others are doing it and maybe change it and apply it to our needs. ANA has been around since before 1911. ANA represents all Registered Nurses today and makes sure that there is an adequate amount of nurses that are highly skilled and well educated. They not only make sure that the healthcare system is getting the most up to date and accurate skilled nurses, they are a true advocate for nurses in the work place as well as the general public. I agree with you that the Nurse Executive (NE) contribute to senior leadership by giving nursing a voice and oversee many functions in the organization. The NE has an important role to play in health care decision making. They have the ability to advocate for implementation of evidence based practice (EBP) in improving patient care while maintaining the vision and mission of the organization. Clinical nurses faces many challenges to use the EBP and that where the NE intervenes since they have the persuasive and decisional power to make EBP utilized within the organization.
There are many stakeholders involved with health care administrations. Those stakeholders can be patients, health care physician, insurance providers, pharmaceutical manufactures, hospital organizations, community clinics and government.
