Event Synopsis/Comments: CBP Recruiters participated in an event; the event was held at the CBP Recruitment Office held at the SFAC on Wednesday, June 21, 2017 from 0800-1600. Supervisory CBP Officer Eric Galarza and Supervisory CBP Officer Sharon Ansick attended the event as recruiters on behalf of the Laredo Field Office along with US Border Patrol Recruiters from the Laredo Sector. CBP Recruiters Ansick and Galarza along with the two USBP recruiters manned the CBP Recruitment Office. Six people, all males to include 5 Veterans, signed the CBP recruitment sign-in sheet. Each potential applicant listened to the OFO recruiters, Galarza and Ansick, give briefings on the employment and application process as well as the duties, benefits, and training.
CCIB received a referral #0424-8590-7518-9033739 via fax from Juana Torres regarding Kenneth Salazar (3) (DOB 5/27/2013). RP reported on 3/14/17, Kenneth was physically abused by his teacher 's aide Rosa Espinoza. RP reported she picked him up Kenneth from his Headstart program and took him home. RP reported Kenneth was wearing a sweater and the she asked him to take it off and he did.
CCIB Intake received a cross report call from Marisa Romero with the County of San Diego CPS. Marisa stated she met with Detective Elizabeth Hundley to initiate the investigation of Gabriel Munoz DOB: 4/6/15 head injury. Marisa stated she and the Detective have statements from teachers regarding the incident. The Director was also interviewed and denied the incident happened. The Director continued to deny the incident even when the teacher Monika Huynh admitted the child hit his head when she fell.
Per the SOC341 on 8/13/15 resident Mullin Bonifacio 14 DOB: 3/23/01 used his foot to fondle 19 year old resident Jonathan Winder 's genitals under a blanket while watching television. According to the report Jonathan 's diagnosed with Oppositional Defiant Disorder (ODD) and other undiagnosed developmental disorders. The report revealed that both residents engaged in the incident mutual and consensually. Resident Mullin is diagnosed with bipolar disorder and intermittent explosive disorder as well as pervasive developmental disorder. According to the report both residents have been separated however they continue to reside in the group
CCIB Intake received an anonymous complaint from a parent stating a teacher named Pam Morgan hit their son Yared Ortega on the head. The incident occurred last week. The complaint was on 9/21/16. According to the complainant the child only speaks Spanish and reported to his parents that Pam hit him. The child is currently afraid of Pam due to her physical behavior towards him.

CCIB Intake received a SOC 341 regarding an incident which occurred on 5/12/16 involving client Christian (last name unknown) and client J.W. According to the reporting party (RP) Program Manager Jim Kaing at 3:10PM while Christian was in the kitchen eating a snack he suddenly rose from the kitchen table and walked into the "Art Room" and pule the hair of client J.W. According to the RP Christian pulled the hair with two hands resulting in some hair loss and a pin sized blood spot on the her scalp. Brenda prompted Christian to have seat and the RP requested Christian to apologized to client J.W (which is consistent with his mother 's request due to his understanding of the nature of his behavior). CCIB received a call from Shauna. Her son Kai Crupt (DOB 10/05/11) attended this day care. Shauna reported that this summer her son came home and told his parents that teacher Yolanda pulled his hair. They thought maybe it was an accident. A few weeks again Shauna got off work early and picked up her son (Around 1:30 PM).
On 7/4/2017 at 2335 hours, Taylor James will be given a 22 hour lock down for violating R-C2-14 for disorderly conduct. Taylor was asked to step out of cell #5 in Booking. I advised Taylor I was walking him back to his unit. Taylor said he wanted to use the phone. I told him he couldn 't until he was processed for his new charge.
Dear Delta Dental Contracting Dept, I am writing this letter inquiring on the status of several credentialing applications that were sent in during the second week of June. The name of the Dental Practice is DAMC Dental & Medical Resources LLC. The list that I have provide below is the name of the dentists who 's credentialing applications were submitted and two of them were already credential with Delta. I just needed to have them added to both of DAMC locations. I under stand that it takes 60 days , however I still would like to see if the contracting dept at Delta has started on and of the applications.
The worker contacted Crystal Thorton; who is a friend of Chetara Washington. The worker asked Mrs. Thorton if she had any concerns for Mrs. Washington and/or family and she stated “No Chetara is a very good mother and a very capable parent. She very patient and you have to be with two children so close in age, but I’ve never seen her yell or curse at her children. CCIB Intake received an incident report from reporting party, Cesar Daproza Case Manager. It was reported that on 6/14/16 at approximately 9:30AM Activities Coordinator Karee Montesanti was driving the van south on Lakewood Blvd when another vehicle swerved into her lane and hit the van knocking off the on the rear passenger side. The driver immediately parks the car and checked the clients for injuries. No injuries were noted. The driver than call 911 reported the incident and reported the incident to the Program Director Gustavo Palomares.
The reporting party (RP) stated on 9/13/16 former foster child Crystal Jimenez disclosed while resident in the foster home of Helen Torres, she would lock the foster child out of the house. The foster child would be required to "calm down" before being allowed into the home. The reporting party (RP) stated there was a bed bug infestation in the facility two weeks ago and resident Joel Lopez-Corrujedo Jr. had bed bug bites on his arms which resulted in a visit to the doctor. His treatment included a prescription for ointment. In addition the RP states there is no hot water in the facility due to the hot water heater being broken since 11/17/16. According to the RP the minor residence are required to use the recreation center nightly to shower.
