Attempt CC =1 CM spoke to Johnda Neazer (DCP&P Supervisor) regarding the family’s current living situation. DCP&P supervisor provided CM with a detailed background on the family and noted that DCP&P will assist the family with housing such as the first month rent and security deposit once caregiver finds housing. DCP&P supervisor explained to CM that the division is unable to place the family into a hotel due to Welfare offering the family to stay at a shelter in Newark and caregiver denied offer. CM was informed that DCP&P only place families in a hotel if they are homeless and does not have assistance such as shelter opportunities. DCP&P supervisor stated DCP&P assisted caregiver with a budget form.
CCIB Intake received a SOC 341 from reporting party Sean Ahern regarding Chris Rueda and Katler Paige. According to the reporting party on 3/5/17 at 10:15am Paige became upset after speaking with her mother and began screaming at Chris attempting to get into his room. Paige threatened she was going to write "fuck you" on his door. He opened the door and Paige grabbed his hand digging her fingernails into his skin. Chris pulled away causing Paige to scratch him on his left hand resulting in an inch laceration along with another smaller scratch.
Per the SOC341 on 8/13/15 resident Mullin Bonifacio 14 DOB: 3/23/01 used his foot to fondle 19 year old resident Jonathan Winder 's genitals under a blanket while watching television. According to the report Jonathan 's diagnosed with Oppositional Defiant Disorder (ODD) and other undiagnosed developmental disorders. The report revealed that both residents engaged in the incident mutual and consensually. Resident Mullin is diagnosed with bipolar disorder and intermittent explosive disorder as well as pervasive developmental disorder. According to the report both residents have been separated however they continue to reside in the group
Attended by Since his 9/9/16 Behavior Clinic visit, Mom reported Ben attends BTC/Day program; walks and exercise daily; She’s counting calories, Ben is still working (picking up paper); still going on community outings with a behavioral residential aide (Addus Health), appetite good, sleeping well and having regular BMs; CSS received copies of Ben’s BATC 10/1/16- 12/30/16 behavior data which show targeted behaviors but no reports of major behavioral issues and 16 absences due to illness. He had 2 reportable incidents per Therap (10/10/16 Physical Aggression & 9/22/16 rectal digging). On10/12/16 he had a visit to American Family care for an ear infection, 10/5/16 had outpatient surgery regarding excessive wax in his ear, seen by his PCP on10/5/16,
CCIB Intake received a call from Co-Complainant Adrienne Williams (323) 802-5315 who disclosed that the son of resident Eleanor Lanzarotta in room 204 punched resident David Lester in the face. Notably David is described as having dementia with aggressive and combative tendencies. Today, April 24, 2017, David entered Eleanor 's room and struck her which resulted in her son striking him back. The complainant was not aware if David sustained an injury from the encounter due to it being the end of the shift. The complainant reported David is not appropriately placed in the facility and poses a significant risk to the other residents in care.
CCIB Intake received an anonymous complaint from a parent stating a teacher named Pam Morgan hit their son Yared Ortega on the head. The incident occurred last week. The complaint was on 9/21/16. According to the complainant the child only speaks Spanish and reported to his parents that Pam hit him. The child is currently afraid of Pam due to her physical behavior towards him.
CCIB Intake received referral #0651-1009-4066-0071107 dated 10/30/16. The reporting party (RP) was Oneil Malcolm, Facility Manager. The RP stated on 10/30/16 while supervising minors Devante Pinson and Jacob Sigala, Devanet was observed taking Jacob 's phone charger and walk towards his room. Jacob was observed stepping in front of Devanet asking repeatedly for him to return his charger. As Devante walked away he moved Jacob out of his way using his left hand.
Mrs. Navarro asked the CP if the CP can provide her with information about entering the shelter, at which the CP agreed to do so. Mentoring Program: The CP provided Mrs. Navarro with information about SCR and mentoring programs for Tyler to explore. Mrs. Navarro reported that she enrolled Tyler and Sidney for the PAL afterschool program, but Tyler refuses to attend-so, the child’s application is placed on hold, according to Mrs. Navarro. Tyler was questioned as to why he is not going to the after school program and based on the child response, he could not provide a proper answer.
Mr. James Kimball, at 23 years old, was a school bus driver who pursued a 15 year old student he’d known for two years. Eventually their “flirtatious” relationship escalated to a “single-sexual encounter” on or about October 3, 1991. The relationship was brought to light by the parents of the child henceforth urging Mr. Kimball to plead guilty to one count of statutory rape in 1992, where the judge then imposed a withheld judgment with a three year probation. After that, Mr. Kimball was accordingly put on the Sex Offender Registry. Due to the essence of the given facts and case report, the disparity in time from the crime to the present, and the technicality of the psychosexual evaluator’s report, I would not have aired the story on Mr.James Kimball.
CCIB received SOC 341 involving resident Camille Griffith (DOB: 03/09/84). Resident left a message with North Los Angeles County Regional Center (NLACRC) supervisor stating that her roommate, Patricia raped her. The resident later stated that it was consentual and then changed her statement to not consentual. Last week, the resident stated that she engaged in consentual sex. The story as reported by resident is that she slept with her roommate 's boyfriend.
Intake Support Coordinator met at the home of Ms. LaShawn Moore for the intake transfer meeting. Tov was energetic as he was playing, interacting with his mother and eating a snack. Tov will celebrate his 5th birthday next month. He was in preschool briefly while the family lived in Hampton but due to Ms. Moore 's work schedule he now attends daycare.
Reporting party (RP) stated that after visiting with her client, resident Bruce Thompson (DOB: 06/24/60), RP removed the resident from the facility due to severe bed bug sores that were bleeding. RP stated that she has not seen a case of bed bug bites this severe. RP stated that they left the resident's belongs at the facility along with his medication. RP stated that the caregiver/facility (Caregiver information is unknown to RP) would not relinquish the resident's medications to him. RP stated that the resident paid $1,000.00 per month to resident at the facility.
Gerald Gault was a 15 year-old accused of making an lewd telephone call to a neighbor, Mrs. Cook, on June 8, 1964. After Mrs. Cook filed a complaint, Gault and Ronald Lewis, were arrested and taken to the Children’s Detention Home. Gault was on probation at that time, after being in the company of another boy who had stolen a wallet. At the time of the arrest of the phone call incident, Gault’s parents were at work. The arresting officer at no time made any attempted to notify or contact the parents of Gault’s parents to inform them of his arrest.
In conclusion, as a future teacher, it is important to understand my responsibility to report any suspected child neglect or abuse. Even if my future student does not give me permission to report the incident, if the child is in danger, it is my responsibility to report. By keeping his promise of confidentiality to J.D., Dr. Pesce put J.D. in great danger. The student was in danger at school because of the misconduct of the teacher, and he was in danger outside of school because of his shame and suicidal thoughts and tendencies.
