They should be hold accountable for any breach in protocols. • Present format for electronic documentation does not allow for comprehensive clinical documentation during follow-up visit. Efforts should be made to upgrade the electronic medical record system to the standard of that expected for a medical center and research institute. This is to allow for proper documentation according to the industrial standard, and easy retrieval of patient’s information for clinical research. There is a need to employ a clinical documentation improvement specialist (CDIS) in this
CMO continues to meet weekly and as needed with division leaders to identify issues and factors that need to be addressed in order to ensure the appropriate operational approaches that should impact clinician as well as client satisfaction and therefore better outcomes. 1. Ongoing in-services for our prescriber staff in the use of our Electronic Health Records (EHR) continue to translate into improvement of the required content in order to justify appropriate billing codings to enhance our collection rates. Chief Medical Officer has personally being reviewing a random number of cases per provider and meeting with them individually to provide feedback and improve their performance. This should also impact obtaining the documentation needed for appropriate coding and improved collections.
When examining a group of physicians who saw ten or more patients during the study, it was revealed that there was a slight significant difference in admission rate from pre-to-post. When evaluating the evidence of this article, it was stated to follow Syncope recommendations and has a high level of consensus. No patient partnerships were disclosed. This study took place at Mount Sinai Medical Center, an academic medical institution. Leadership roles were present and there was audit and feedback throughout (Melnick et al., 2010).
He effectively communicate through verbal, non-verbal, reading, writing and listening in persons and in groups. Also, he effectively interact with patients, families, colleagues, and other healthcare professionals with sensitivity to cultural and ethnic
Overall, incomplete documentation and delinquent medical records cause inaccurate reimbursement and results in inaccurate gross revenue to the hospital. It can have a negative impact on the hospital budgeting and financial planning process for the hospital. It is for this purpose that every healthcare institution should be purposeful on reviewing the accuracy and completeness in clinical documentation, no matter the cost. Even though, for most physicians, most of their time is focused on the actual care of the patient and there is little to no time to devote to extensive documentation, it is imperative to understand patient care includes both the one-to-one attention and the documentation of said treatment.
Based on FFA, it is recommended that in order to emphasise on a sense of a common group identity among diverse staff, RGO’s policies should be securing their future and making them feel that they are trusted and truly valued. Also, any source of discrimination should be eliminated, for example, the employment contracts should be unified for all, including expatriates to create a sense of fairness (Nishii and Özbilgin, 2007). Also, selection criteria for training and promotion should be equal. More authorisation and autonomy should be allowed to the RGO Medical Director. Also, it will be more appropriate for the leader of RGOMS to be a military-trained physician (Kirby et al., 2011) and to get appropriate leadership training to show more support
I am excited about the opportunity to share my insights and work with peers and faculty members to create positive change in the healthcare field. Comparatively, during my internship with the assessment team at California University of Science and Medicine, I was given the incredible opportunity to understand administration, team management, and technical skills comprehensively. Working with the medical school's software systems, I organized and delivered student assessment activities and information. In order to communicate effectively with delivery teams, I learned to collate large datasets and present them
Abstract This paper presents an overall idea about the main errors committed by medical interpreters. According to Abrue et al (2010), five main errors were noticed to be mostly committed by medial interpreters: omission, addition, substitution, editorialization and false fluency. Moreover, according to Flores (2005), these errors affect the quality of health care, which is categorized into three main sections discussed in details. Additionally, the paper shows the three types of medical interpreter, and how they are related to the proportion of errors committed.

As an aspiring physician in Emergency Medicine, it is hard to describe typical physician workday activities. Every day is unique and filled with many opportunities to learn and develop clinical, interpersonal communication, leadership and critical thinking skills. Despite this, there are certain routine activities which I had the opportunity to observe through my shadowing experience in the Emergency and Operating rooms at California Hospital Medical Center, Los Angeles. One day, a two year old patient came into the ER after falling and cutting her head. The patient’s mother told the physician that her daughter is nervous and scared.
Submission of related publications was also encouraged” (Columbia 2017). To hone in on these requirements I wanted to demonstrate that I had done some unique investigative work that would contribute to important observations that are not so easily accessible to those without such experiments. Since the whole premise of narrative medicine is combining a passion for medical patient care, phenomenal written communication skills, as well as respect for the patient and human psychology, I thought it was important to discuss my vast experiences in the intimate setting of doctor patient consultations. I also made sure to mention the Plan II Honors program, because of its similarities with the Columbia core program to promote an interdisciplinary approach to learning. The fact that Dr. Rita Charon, the executive director of the program chose the Plan II Honors program to deliver a talk to, which involved flying over from New York, suggests that she has some interest or respect for the program and so it is worth starting the note off by mentioning
As we work in the society, we are automatically associated with the well-being of the society and the people. I need to take responsibilities and have knowledge about what is going on with my patients and help them to be actively involved in their daily activities. Being able to communicate effectively with the people is the biggest key aspects of our daily life. I have to use the communication skills through reading and writing in my career which helps me to build trusting relationships with my patients. It will help me to understand the effects of the therapeutic process for the benefits of the patients.
