I think I have learnt many skills and knowledge this semester. Among all different aspects, I take several key points that will help me to manage and working with clients with a range clinical conditions into the future. Firstly, I have learnt that when assessing and managing patients with a range of clinical conditions, it is important to find out what is of priority to the patient in order to provide them with the best treatment, so prioritization is the vital skills to set the best goals and objectives in the nutritional management plan. In addition, I have learnt that using comprehensive assessment including biochemical tests, medication, clinical sign , anthropometry, diet history and social history will enable me to identify the prioritization …show more content…

In general, a appropriate approach can help build up the rapport with patients and enable to set out the management plan with common goals easily. Finally, I have learnt that making a plan for the consultation based on the patient’s past medical history, referral letter from other professional and past assessments, which help the consultation will be structured and make it more focused on what you need. I have found throughout simulated patient workshop, in consultations with simulated patients and by watching my peers that can make me have more ideas for the management plans about the case studies and learn how to be more client centred and appropriately targeted the consultation from my peers. What do you think/feel you personally need to develop further in strengthening your work in individual case management and what are some specific strategies you can implement to assist you? I believe that making a structured plan for the consultation based on various result of past assessments that is important procedure prior to consult with a patient.
