I became more aware of the importance of documentation and records as a requirement of third party payers, educational purposes and researches .This obviously decreases the chance for malpractice.
Actually,regarding spinal cord injured patients, I used to refer them as a paraplegic or quadriplegic, but after this course, I replaced these terms by (a patient with a spinal cord injury at level......resulting in .......)this had a positive feedback from physicians, patient and families
I sought to ensure that documentation on patients throughout the period of care should be completed in a timely manner and accurately describe the patient 's status, management & intervention and outcomes. I aimed to limit the use of abbreviations as much as possible.

Furthermore, I seek consultation if uncertain, keep the referring physician informed and do not delegate to unqualified practitioners. I also keep an accurate and timely written records. I tried very hard to apply what i have learned when I became a senior in musculoskeletal section in my workplace, so when I supervise the physical therapist assistant in any off-site settings, I ensure that I 'm accessible by any type of communication while the physical therapist assistant is treating patients, even if I’m absent. In some situations in which the assistant is delivering a service to the patient, I do a supervisory visit regularly according to a timetable, the needs of the patient or upon the physical therapist assistants request for reexamination, when a change in the plan of care is needed, before any discharge, and in response to a change in the patient 's status. I 've become more aware and respectful for standards of practice for physical therapy, including the ethical and legal considerations, policies and procedures, and administration. I ensure that services are consistent with the mission, purposes and goals of the physical therapy service as well as are provided in accordance with established policies and procedures.
