Obtain patient’s self-report of pain including, location, onset, duration, and rating on a pain scale of 0-10.
This nursing intervention is necessary because the patient self-report " is the most reliable indicator of pain"(Treas & Wilkinson pg. 1096) along with a comprehensive pain assessment. The nurse needs to understand the patient perception of pain to treat the patient pain effectively. It is evident that both of these individuals are experiencing pain and having difficulty expressing it to the nurse.

1.Would be assessed for nonverbal signs of pain. The rationale for this would be that patient cope with pain differently. You can compare a patient verbal report of pain along with physical signs and symptoms they are presenting with and can question the difference.
