Kaiser Permanente has been equipped since 2007 with Health Connect; which is the largest private electronic health record implementation in the world. This is a highly sophisticated electronic program that integrates inpatient, outpatient, and clinic medical records with appointments, registration, pharmacy, and billing for all kaiser members. In addition, this electronic program includes an entire medical library with a whole set of care support tools which are accessible to doctors, nursing staff and patients (Kaiser Permanente, n.d.). At kaiser permanente; nurses are expected to print out “the after-visit summary” (AVS), which contain the doctor recommendations for each patient that we see.
They should be hold accountable for any breach in protocols. • Present format for electronic documentation does not allow for comprehensive clinical documentation during follow-up visit. Efforts should be made to upgrade the electronic medical record system to the standard of that expected for a medical center and research institute. This is to allow for proper documentation according to the industrial standard, and easy retrieval of patient’s information for clinical research. There is a need to employ a clinical documentation improvement specialist (CDIS) in this

Overall, incomplete documentation and delinquent medical records cause inaccurate reimbursement and results in inaccurate gross revenue to the hospital. It can have a negative impact on the hospital budgeting and financial planning process for the hospital. It is for this purpose that every healthcare institution should be purposeful on reviewing the accuracy and completeness in clinical documentation, no matter the cost. Even though, for most physicians, most of their time is focused on the actual care of the patient and there is little to no time to devote to extensive documentation, it is imperative to understand patient care includes both the one-to-one attention and the documentation of said treatment.
We have all seen a doctor or nurse at some point in our lives. However, when I need to see a doctor the last thing I think about is the paperwork. In this day in age doctors and nurses spend most of their time on paperwork. Rather than focusing their attention towards the patient. The documentary Code Black directed by Ryan McGarry shows the struggles that many Emergency Room nurses, and doctors face in a community hospital.
It is my responsibility to have admission and continued reviews done promptly and accurately to determine whether the patient meets medical necessity for inpatient or outpatient status. I review all patients’ charts, especially those in an observation status. They are reviewed frequently and in real-time to ensure they are actively being managed and in the correct status. Observation patients are given priority in order to catch patients that may be discharged. Collaboration continues with the physicians and if there is a question about status, the chain of command listed above is followed.
The following scenario will best reflect my practice and use of informatics. The scenario is not representative of a particular patient but is a combination of daily events in my position so that no patient rights are violated. I am three hours into my shift as the assistant nurse manager (charge nurse) of a busy emergency department (ED) with my responsibilities in the department being to manage the flow of a shift that will see roughly 100 new patients during the 12 hours but also oversee the care of the 5-20 long term patient who are listed as observation or inpatient holds. We can expand to 60 beds with the use of hall beds. I have a bank of monitors to my left which display the EKG and vital signs of over 48 patients.
Electronic Health Records and Patient Confidentiality Technology has become an essential part of our everyday life therefore, it makes sense that doctors and hospitals get rid of the old fashioned paper charting and use technology to access patient records. Electronic health records (EHR) provide quick access to information, as doctors no longer have to wait for other providers to fax previous records to them. The accessibility of Electronic Health Records assist medical providers to make quick medical care decisions, by accessing previous care provided to patients including treatment and diagnosis. Quick access to information through EHR enables health care providers to treat patients faster as there is no need for records to be mailed or
Before answering the question of “Should physicians pay attention to “race”?” lets first discuss why it is necessary for a physician to know the “race” of the patients. The first reason is in regard to pathogenic reasons leading to the field of pharmacogenetics. The second is more social reason and leads to racial division among a population. In considering of the latter reason, one should note that it may not the physician’s intention to cause any racial division.

This information is used to appropriately implement prevention and treatment for patients. The second outcome integrates analysis of information gathered by healthcare personnel to identify trends and inconsistencies within the healthcare population. Through this the origin of problems can be ascertained, and preventive measures can be instituted. Subsequently prevention will decrease incidences and ultimately the cost to
As the famous political theorist Niccolo Machiavelli once recognized in his most well known work, The Prince, it is never easy to create an entity of new origins. The Initiation of a new order of things would best be interpreted and depicted as the establishment of a governing entity. Circumstances of this nature could include either altering the already existing governing entity, or through the creation of a governing entity. The ways in which a country has established itself have greatly varied from different cases. However, many have contained a united front often in unison with a catalyst in the form of a figure head.
