Open disclosure is open discussion about incidents that happened during care which caused harm to a patient, with the patient, their family, carers and other support persons. This standard includes the following principles:
1. Openness and timeliness of communication- In case if the things go wrong, it is the responsibility of the health care practitioners to inform it to the clients clearly in an open and honest way.
2. Acknowledgement- The health practitioners should acknowledge all the adverse events to the patient. It is necessary to acknowledge the adverse events which occurs and open disclosure process should be initiated.
