2. During my placement I have completed various types of assignments and research. When doing my agency profile I have researched the policies and standards of Extendicare. I have also co-facilitated in programs such a pong cup, fishing game an beauty session where I worked with the recreation staff and demonstrate my facilitation skills to help the group members of these activities have a fun session. When being a co-facilitator I have an opportunity work with another facilitator such as Chantal or James. Facility members attend meetings regularly here at Extendicare, and I was welcomed into most of them. Attending the RAI meetings every Monday, Tuesday and Thursday has help my knowledge of the psychosocial needs of the resident on each unit. I have also done quarterly assessments and documentation for these meetings. Another meeting that I attend every Monday is the “High Risk” meeting, this meeting is to discuss the residents who are at high risk either by: falls, nutrition, wounds, equipment, pain, behaviors, wounds, isolation, medical, sleep, palliative or medication. All of these categories if affected can impact a resident’s well being, so these meetings are help to discuss the issue and set goals for a plan of action with then follow up with the families or other health care aids. I have discussed these meetings in my journals and the interest I have for them. This meeting allows me to enhance my recording and observation skills and my goal setting process. In Nathalie department, there is the MDC’s meetings where resident and their families either come to the facility or call in to discuss how the resident is functioning and is the family has any questions are concerns about their loved …show more content…

I believe that my placement at Extendicare has given me many learning opportunities, where I have succeeded. When I was first thinking about my second year field placement my main choice was to gain experience with youth or adolescents, and was a bit unwilling to gain experience in long-term care workplace. However I have enjoyed my time here at Extendicare, I have learned so much and have accomplished a lot. When it comes to Crystal’s department, I have grasped the concepts of the admission process as well as her networking agencies such as CCAC, The Hospitality Network and The Ministry of Long Term Care. I’ve accomplished in providing tours to families and potential residents who are interested in the facility. I have built different relationships with many of the residents here at Extendicare. These relationships are all different an unique, for example with one resident is it a very joking, laughing relationship where we have the trust to pull fun at each other while either playing bingo, Wii or just talking. This relationship is different then the one where I have with one other resident, where he teaches me how to play different kinds of games, and there is a trust where if he needs help he came come to me for help or just to talk about his day. I have attended various types of meetings and gain knowledge in my recording and observation skills. When it comes to spending time with residents, I have practiced my social service worker skills, and goal setting
