Having worked in utilization management and case management in an inner city hospital in Camden, New Jersey brought many realizations regarding social determinants to the table when trying to find optimal discharge planning in not so optimal situations. Social Determinants is a new initiative for Healthy People 2020 (Office of Disease Prevention and Health Promotion [ODPHP] 2016.). Where we live, how we live and the conditions in which we live can affect our health and well-being leading to chronic conditions. In my community of Camden County, New Jersey, health trends and disparities were evaluated with findings that revealed an 11.3% poverty level in families, and an 8.9% poverty level in families with children. It is of particular interest as poverty plays a role when we focus on looking at the relationships of communities, housing, food, education, our schools, safety and transportation and its effect on chronic disease as well its role in our discharge planning. The goal is to promote healthy social and physical environments that will subsequently promote better choices leading to the promotion of health. As a case manager, it is imperative to get to know the patient, their background, how and where they live and the barriers that they are faced with for optimal health. Living in a county that encompasses the city of Camden, it is important to understand the relationship of poverty and disparities that affect ones health while reaching for solutions to promote Healthy People 2020 and stop the vicious cycle of social …show more content…
