Attempt CC =1 CM spoke to Johnda Neazer (DCP&P Supervisor) regarding the family’s current living situation. DCP&P supervisor provided CM with a detailed background on the family and noted that DCP&P will assist the family with housing such as the first month rent and security deposit once caregiver finds housing. DCP&P supervisor explained to CM that the division is unable to place the family into a hotel due to Welfare offering the family to stay at a shelter in Newark and caregiver denied offer. CM was informed that DCP&P only place families in a hotel if they are homeless and does not have assistance such as shelter opportunities. DCP&P supervisor stated DCP&P assisted caregiver with a budget form.
Arrested after 36 patients died, Narendra Nagareddy had been held at his office following a raid from DEA agent. Around 12 of the 36 patients died from an overdose. Almost 40 federal and local agents raided his Jonesboro office as they seized even more assets at his home. CCIB Intake received a call from Co-Complainant Adrienne Williams (323) 802-5315 who disclosed that the son of resident Eleanor Lanzarotta in room 204 punched resident David Lester in the face. Notably David is described as having dementia with aggressive and combative tendencies. Today, April 24, 2017, David entered Eleanor 's room and struck her which resulted in her son striking him back. The complainant was not aware if David sustained an injury from the encounter due to it being the end of the shift. The complainant reported David is not appropriately placed in the facility and poses a significant risk to the other residents in care.
CCIB Intake received referral #0923-2901-5102-8047131 from reporting party (RP) Katie Hacker Sierra Child and Family Services staff member. According to the RP child Shawn Williams age 10, informed a group home staff member (name not provided) on the morning of 9/26/16 that during the night his roommate Arthur Hammett age12 had touched him in his "inappropriate parts." Shawn failed to provide any specifics regarding the incident and the RP didn 't question him due to his frequent fabrications as of late. To the RP knowledge, Arthur has not displayed this type behavior before, however Shawn has a history of sexualized behaviors which has been previously reported to CPS.
worked for the Children’s Center of Cicero Berwyn as a part-time Home visitor for six months from September 2016 thru March 3rd, 2017. She was a good employee who had passion to work with children and families. She created lesson plans for five children and families, prepared developmentally appropriate activities, observed children, provided resources and referrals to families in the communities. Her doctor had instructed accommodations and those were in place for her. Those accommodations included not to sit or stand for prolonged periods of time and not to lift more than 10 pounds.

RP called to report that a program participant of the Resource Center Candice McGurre (435-899-9390) called to stated that her children that attends this daycare told her that Ruth has slapped children on the hand and also shoved a child against the wall. The last classroom there was one caregiver Juanita O'Hair caring for four children ages 4 months to 7 months. One child was asleep in their porta crib, the caregiver was holding one child while feeding and other two were on floor playing with toys. The child who was asleep woke up and was immediately removed from the crib and placed on floor with other children. All rooms were measure at inspection as the director is wanting to spilt the infants into two room for older and younger infants. On 08/28/15 Steven came to school covered in live bed bugs and bed bug bites; this is the second time this has occurred. Steven told the reporter his grandmother treated the bed bugs on Monday but there is still an infestation and he wakes up at night with bugs on him. The family is poor and in need of resources but the child’s basic needs are met. Steven doesn’t require any medical treatment.
