Mr. and Mrs. Blakeney (MHS) describe Loshane as hyper and playful. MHS report Loshane does display verbal aggression towards peers and adults. Loshane display physical aggression, however use a bat to hit the tree to release the aggression. Loshane continues to have negative and positive peer and adult interactions.
Loshane is not involved in any extracurricular activities now. Loshane works at the local Sonic’s restaurant. Loshane hopes to increase his working hours. Loshane attends community events with MHS and family.
Per referring agency, Loshane biological parent’s parental rights are terminated. Loshane does not have siblings in care.
Loshane is expelled from public school. Loshane started an adult education program to earn his GED. Loshane reports adult education …show more content…

Loshane is accountable for keeping his living area clean and has weekly chores.
Loshane was seen at Palmetto Primary and Specialty Care Physicians for all medical needs. There no history of developmental, emotional or behavioral issues. Loshane does not have a history of illnesses.
Loshane did not display age appropriate behaviors. Loshane display behaviors of a 14 age old male. Loshane is hyper and playful. When unable to gain permission to do things, Loshane gets verbally aggressive towards peers and adults. Per MHS, Loshane has recent hanging with negative peers. There is concerns about cannabis use. Loshane does not display maturity constantly.
Loshane does not have a visitation plan. The referring agency will schedule and supervise all face to face visits with the biological family.
The referring agency will provide follow-up for individual therapy and substance abuse assessment it needed.
It is recommended Loshane receive daily Psychosocial Rehabilitative Services (PRS) and weekly Family Support services (FS), individual therapy, medication management and substance abuse
