On 10/29/2015 SO EMT Perez was dispatched to PV-119 regaurding foot pain. SO EMT Perez knocked and was verbally greeted in by the resident a Mrs. Suzanne Truss. CCIB received an Incident Report from the LTCO Maged Mansour (213) 351-6433 regarding resident Maria Alcantara. The SOC 341 states that Maria disclosed to facility staff and her Day Program staff that residential facility staff member Gloria G hit her in her back. On 3/17/17 at 1005 hrs, I was dispatched to the PD lobby in reference to harassing phone calls. Upon my arrival, I made contact with the complainant Victoria Wert and her boyfriend, Bradley Palmer. Wert advised Palmer’s ex-girlfriend, Alexus Smith has been texting her since 9/27/16 to 3/17/17, during this time Wert advised she did not text her Smith once, and her text messages are rude and unwanted. Wert advised she did not receive any concerning messages until 3/17/17 at 0930 hrs.
On 7/19/2015 SO EMT Perez was dispatched to CC-501 regarding a re-bandaging of a pre-existing room. S/O EMT Perez knocked and announced his presence at the door and was verbally invited in by the resident. Mrs. Betty Robb stated that the bandage that she had over a skin tear on her right forearm had fallen off and would like for it to be re-bandage. S/O EMT Perez provided re-bandaging for Mrs. Robb. I responded to Troy Regional Medical Center in reference to an assault call. Upon arrival, I made contact with Roseline Jackson in room 9. Jackson stated that her and her baby daddy got into a physical altercation around 13:00 on this date. Jackson stated that she was picking their son up from Bean mother house and that’s when Bean asked to borrow her car. Bean got mad because Jackson wouldn’t let him use it.
I responded to 500 Trojan Way in reference to a harassing communications call. Upon arrival, I made contact with Amy Evans. Evans stated that her son(Richard Davis) baby mama keeps texting her phone, threatning her. Evans showed me the text messages which stated the offender(Queisha Carlton), was going to stab her son and slash the tires on his vehicle next time she seen him. Carlton also stated that she was going to bust every window out.

10mins later BN StaffDuty comes to my room to inform me you said to SFC the NCOIC of staffduty "you suck and fuck off and or fuck you. StaffDuty told me this was the last time he better no come outside of we are going to have to call 1SG, SGM and BC. I then go to get you downstairs again and which you did not want to come up to your room after you said sorry to StaffDuty you then did not want to go in your room and this point i allowed you to seat on the end of the hallway to get air in the start well exit door. You told me to "don't fucking touch me I dont need you or your help fuck off" After repeatedly ordered you to remain in your room and remain calm over and over again. Your failure to obey an order and your blatant disrespect towards the leadership of this unit and this PLT will not be tolerated.
My resident was an older Caucasian female with diagnosis of dementia, congestive heart failure, iron deficiency, thyroid dysfunction, and several other illnesses. As I arrive to my unit at the start of shift, I had a resident approach me, wanting her medications, while she is standing on her walker, I notice, the resident having some shortness of breath. When obtaining her blood pressure, I remember both numbers, systolic and diastolic, were extremely elevated, I obtain and 02Sat, which seem to improve, after a few minutes of resting. I continued by asking is she had any headache or chest pain? She continued to state, yes for the headache, but no on the chest pain.
