My field experience with Vidant Home Health and Hospice was rewarding and challenging, but overall it was such a privilege to work with patients and their families at the most vulnerable times in their lives. I intern with the social worker and the Chaplin to know how the social work part works and the bereavement part works. I learned to use evidence-based practice and psychosocial evaluation to make sure that I was able to give the patient and the family the best care so I could help increase the quality of life and provide the patient a positive environment. While I was interning in hospice, I had to to make sure the patient is comfortable, free of pain, provided emotional comfort, make sure their mental state is okay, and make sure to supply their spiritual needs according to their culture. I was not sure that interning at hospice was the right thing for me because it was numerous deaths. As I began to work more with the patient and the family, I became more at ease and more dedicated to helping, listening and learning about hospice and the patient on a more profound level. I was able to relay information about the patient to the interdisciplinary team and come up with a plan that would assist the patient, and their loved ones, through this challenging time of end-of-life-care, and to make this difficult situation as smoothly and stress-free as possible for the family. Working with The Chaplin gave me the opportunity to become a spiritual counselor, to actually create a bereavement plan prior to death. After the loved one has died I make phone calls to the families to see how they were dealing with the loss of their loved ones. I helped set-up a bereavement group for families to talk openly about their loss and how difficult it was to move on. I help set-up shops for families that were called memory bear, where families could bring a piece of their loved one clothes and make teddy bears out of
