On 10/29/2015 SO EMT Perez was dispatched to PV-119 regaurding foot pain. SO EMT Perez knocked and was verbally greeted in by the resident a Mrs. Suzanne Truss. Mrs. Suzanne Truss was very stressed and seemingly overwhelmed and she stated that she had foot pain but before SO EMT Perez had a chance to evaluate Mrs. Suzanne Truss got up on her own strength from her bedside and proceeded to walk to her bathroom with assistance of her walker. Mrs.Suzanne Truss wanted SO EMT Perez present in the bathroom because she stated it made her less nervous. Also, this proves the staff just thought all his signs of not being an idiot were just gestures of an idiot so they had no hope for him
On October 30th, 2015 at 2116 hours, Florida Hospital Security Operation Command (SOC) radioed FHEO Security for a Disorderly Patient ED (51D). Officers Omar Alonso-420, Steven Evans-407 and Carlos Ayuso-415 responded to the call. Upon arrival, Security staff observed Nurse Traci Davila and Dr. William Kotler in Room ED #39 talking and advising patient, Christopher Sheets (MRN: 3028388/Fin: 84773293) about the risk he takes without seeking medical attention. The patient, Mr. Kolter wasn't combative, was just anxious and wanting to leave because he didn't know what happened and also because he didn't have insurance.
Patient is a 51 year old Caucasian female living alone in her own home. The patient has a history and current syntoms of geralized anxity disorder, social phoina and panic attacks, which she takes madications for and sees a psychrist 2-3 times per month in Havasu. Pt has a daughter in Big River, who is not supportive of her, but has three grandchildren who visit her.
Mrs. Clymer is a 58 year old female who presented to the ED with medical issues, however expressed some suicidal ideation per nursing staff. At the time of the assessment Mrs. Clymer was found asleep. She was awaken by QP calling her name. Mrs. Clymer appear 4Xoriented and willing to cooperate.
I enjoyed reading your discussion post regarding bedside reporting and agree bedside reporting is an excellent example of evidence-based practice. The SBAR tool was originally developed to improve communication between nurses and physicians (Cornell, Yates, &Vardaman, 2014). Moreover, given the impact SBAR had on nurse-physician communication, bedside reporting utilizing the SBAR tool is now evidence-based practice for nurses (Cornell, Yates, &Vardaman, 2014). Bedside reporting gives nurses and patients’ opportunity to set mutual goals.
