After a review of all information, including the most current assessment, as outlined below, Healthfirst is changing the services you currently receive. We are approving a total of 13 hours per week for personal care assistance support. You live with your daughter who is willing to provide support. Your recent home assessment was done on 07/17/2015 of your activity of daily living abilities, including your medical history and current diagnoses. A mistake occurred in the previous personal care service authorization. You are an 82-year-old female with a history of Osteoporosis (decreased bone strength), Osteoarthritis (degenerative joint disease), Walking difficulty and Joint pain, which impairs your ability to perform certain functions …show more content…

You need limited assistance with grooming, dressing your upper body and transferring to a toilet. You need extensive assistance with bathing, dressing your lower body, ambulating (walking), locomotion (moving around) and toilet use. You need maximal assistance with medication management. You do not require supervision or total assistance with any personal care tasks at this time. You wear underwear liners and adult size disposable undergarments to manage your bladder incontinence. The assessment indicates that you also need Level 1 support with following tasks such as housekeeping and simple meal preparation. The tasks may include: transportation, routine housecleaning, laundry, and mailing letters, paying your bills; shopping, keeping track of needed supplies and running essential errands, if you have no other arrangements. You do not have a Personal Emergency Response System (PERS) device in the home for emergencies. You do not attend a social or medical day care. This is not a Mutual Case. There is no one else in your home receiving services in another plan. You have not been hospitalized recently. You have not been to an emergency room recently, had a change in where you reside, nor had any major changes in your family support arrangements.
