Wellness Coach Teresa Cain met with Phillip Harris to conduct his bi-annual Service Plan Review, Medical Health, and Mental Health. Phillip Harris scheduled Service Plan Review was on 12/30/2016. Mr. Harris came to the staff office appearing to be alert and groomed. Wellness Coach asked Mr. Harris how was his day and he stated it was well just waiting on a package to complete my home project. Mr. Harris express to wellness coach on several occasions that he is repairing a house in New Jersey. He goes there weekly and is a technology buff. Mr. Harris said he would like to make it a “smart house” meaning that every appliance would need a form of technology to work. Wellness Coach replied that sounds very interesting and it should be successful. Mr. Harris is in the action stage of his goals and those goals are as follows: Manage Depression: Mr. Harris is currently in the action stage of change when it comes to managing his depression. Mr. Harris has express to the wellness coach different ways he identifies the triggers in which causes him to become depress. Those triggers are holidays and not speaking to his …show more content…

Harris is in the preparation stage of change with this goal. Mr. Harris has a degree in visual arts in which he using alongside his hobby of photographer and film development. Mr. Harris currently works with different event planners in which he provides a service to which creates memories for weddings, birthdays, and special occasions. He has express that he enjoys creating stories through film and want to better his hobby by using the skills he learned in school. Mr. Harris express that he is a “technology buff”. Mr. Harris express that provided a gift of knowledge in the visual arts field he could mentor others as well as enhance his hobby of film and art. Mr. Harris told staff that he will like to continue working on this goal in the next six months. Staff will work with Mr. Harris in gaining knowledge where he could advance his visual art
