Attended by Since his 9/9/16 Behavior Clinic visit, Mom reported Ben attends BTC/Day program; walks and exercise daily; She’s counting calories, Ben is still working (picking up paper); still going on community outings with a behavioral residential aide (Addus Health), appetite good, sleeping well and having regular BMs; CSS received copies of Ben’s BATC 10/1/16- 12/30/16 behavior data which show targeted behaviors but no reports of major behavioral issues and 16 absences due to illness. The service user I chose to assess is a 17-year-old Hispanic female whose modified name is Tiana Santos. I provided therapeutic casework services to Tiana for a year in my previous role as a Therapeutic Caseworker at the Children’s Village, a foster care agency. Tiana was placed in foster care after been removed from her mother’s care due to emotional and physical abuse. Tiana’s father abandoned her mother and her youngest sibling when she was 5 years old. Tiana’s memories about her father were very limited due to her young age.
Resident E.V. has demonstrated the ability to cope by displaying effective coping patterns. She has the desire to become both healthier and live a healthier life style. Her goal is to gain recovery so she can walk on her own again and eventually move back into her house to help take care of her husband, whom has dementia. She is willing to modify her lifestyle accordingly if she is unable to gain complete recovery. However, she would like to be as independent as possible.
concerns about depth perception and light sensitivity. there have been no falls but has walked into things and needed reminders to slow down and to walk with staff. she has gone to the eye dr and there were no significant changes in eyesight but will continue to go for regular 3 month check ups. The mother reported that she has two sisters with who she is close to. She indicated that her parents are alive and together. She indicated that she experienced no abuse as a child. She did not experience any traumatic events as a child. She reported she was an average student with no behavioral issues.
Jenifer is a busy stay at home mother of three children (2,4,7). Jenifer has some casual friends that are other mom’s but say she has not had any close friend senses she stopped working. Jenifer’s husband is a physician and was offered a job here in Maine 3 years ago, which cause them to move to Scarborough. Jenifer grew up in home with her mother, brother and step father after losing her father at age three.
Madison was referred for a full psycho-educational evaluation as one part of her mandated three-year re-evaluation process. Madison has a current special education classification of Other Health Impaired, secondary to Neurofibromatosis. She is educated within a general education classroom; in addition, Madison attends resource room daily for 40 minutes in a five student to teacher ratio classroom (5:1). She receives counseling services with a counselor for thirty minutes twice a month. Her testing accommodations include extended time (1.5) and refocusing/redirection.
Description of the Interviewee Kelly Jo Duprey is a Licensed Professional Counselor, who works at Samaritan Counseling Center and Foundations Counseling Center (K. Duprey, personal communication, September 8, 2015). She started counseling in 2001 and got her license in 2003. She has worked at Samaritan since 2006 and Foundations Counseling Center since 2010 (K. Duprey, personal communication, September 8, 2015). Kelly Jo also became a Medical Assistance Certified Provider in 2008 (K. Duprey, personal communication, September 8, 2015). She has 21 years of experience working with families and children and 14 years of experience counseling families and children (K. Duprey, personal communication, September 8, 2015).

For her social life, Annette spends time with her two grandchildren, dines out with her husband, and watches movies with friends (Prizio, n.d.).
During my visit to Oak Crest Village, I had the pleasure of interviewing a retired nurse. To avoid releasing too much personal information, I will refer to my interviewee as Mrs. B. Mrs. B is a 76 years old female born in 1939 right here in the state of Maryland. She started her nursing education through a diploma program at Mercy Hospital. Mrs. B stated that her nursing program was on the job training. She spent little time in the classroom and the rest was all hands on training completed during patient care.
A career as a Psychiatric-Mental Health Nurse Practitioner is a natural extension of my personal, educational, and research experiences. Although my path to nursing has not been a straight line, every experience that put me on this path has shaped my passion and dedication to psychiatric nursing.
