SC met w/ Pa, at EADC Penn Asian Senior Services (PASSi) and completed monitoring visit. Present at the visit was a staff member/Chinese Community Liaison and interpreter name Pearl who interpreter thought-out the interview. The Pa was clean, appropriately dressed in street clothes. The Pa, SC and interpreter complete interview in a private room. The Pa appears to be in high spirits and well. The SC noticed that the Pa was walking steadily without any medical devices. The asked Pa how is her legs feeling and she said better than the lost when she saw the SC. Pa reported no, hospitalizations, ER visits, or changes in medications. SC will continue to provide assistance to the Pa to identify alternative community-based sources of transportation. The SC reviewed the Pa’s needs and goals and they remain unchanged. The Pa states that the services are helping to meet her needs. SC reviewed Pa 's ISP. Pa confirmed that she continues to receive the service plan in the ISP in the following type, scope, amount, frequency and duration: Agency Model PAS 4 days x 3 hrs (Tue, Thu, Sat, Sun: 9am-12pm & 3days x 2 hrs Mon, Wed, Fri: 6:30-8:30am) from Adwa Home Care agency aide PAS. Aide assists with bathing, dressing, grooming, transfers, all meal prep, light housekeeping, laundry and shopping. Pa also continues to receive ERS from Active Aid Solutions, which she uses in the event of an emergency. SC will continue to see Pa based on the agreed upon contact plan of home visits every three months, calls every two months and annual reassessments. The Pa is requesting 1 extra hour on Mon, Wed, Fri to help with meal prepares and personal care. The SC reviewed Self-directed service option, but the Pa declined as she is pleased with Agency Model PAS. The SC believes that one extra hour would provide Pa the right amount of support necessary to ensure her health and safety. However the Pa remains satisfied w/
