Lisa is a very pleasant 16-year-old Caucasian female. She came in for her appointment at 11 AM. Lisa was accompanied to the appointment by her mother, Ms. Amy Burrow. Lisa's mother has a valid drivers license and transportation. Lisa also has a valid drivers license and dependable transportation. Lisa and her family live in Flint, Texas. Lisa was able to walk independently without any type of aid. She demonstrated no unusual or atypical involuntary motor movement, such as tic disorder or seizure activity. Lisa appeared her stated age. She was dressed appropriately. She wore a checkered longsleeve shirt, black pants and black tennis shoes. She had adequate hygiene. Lisa was very cooperative. There are no reports of initial insomnia, intermittent insomnia, or early wakening insomnia. Lisa wakes up around 8 AM in the mornings. She wakes up with her on alarm. She is not sleeping in the mornings. She is described by her mother as being happy in the mornings. There were no reports of bad dreams. There are no reports of sleepwalking episodes or night terrors. There are no reports of enuresis or encopresis. Lisa is described as having a very good appetite. She likes burgers. Her favorite place to eat hamburgers is "Steak and Shake." Lisa is right-handed. Lisa was dressed in a clean and neat manner. She had a good social smile. She was able to provide good direct eye contact. Lisa appears to have very good energy. Lisa does not report any current symptoms of depression. She does not report being easily angered or irritated. She does sometimes get aggravated with her sisters. Lisa's mother was question regarding any history Mood Disorder, Bipolar Disorder, or ADHD in the family of origin. Lisa's mother reports that on her husband's side there is a history of mental illness and Bipolar Disorder. Lisa's mother reports that she has struggled with some depression
