Gurniack, Lt. Kunak heard yelling and screaming from the cell area. Lt. Kunak went to the cell area and observed P throwing himself against the cell and spitting. Lt. Kunak got a taser and told P to calm down or he will be taser. Lt. Kuak opened the cell door and tasered P. An ambulance was called and P was taken to the hospital.
On 09/18/2016, at approximately 2148 hours, at the Durango Jail located at3225 W Gibson Ln Phoenix, AZ 85009 an Inmate/Inmate fight had occurred in Durango 3 B Pod between Inmate Moothery, Naz T303229 and Inmate Sams, Worine T284566. Upon entering Durango 3 B Pod, Inmate Sams approached Inmate Moothery and begun fighting by Durango 3 B Pod door. Multiple strikes were thrown by both inmates until Inmate Moothery had fallen to the Pod floor. At that time Officer Serrano approached the pod door and gave both inmates commands to stop fighting. No radio call was made due to both inmates complying with the orders given.
On 11/07/2017, the agent received a Law Enforcement Contact form indicating offender Joe A. Slugger was at the K&D Tap on 11/06/2017 having unauthorized contact with Sandra Williams. It was stated in the LEC form that Joe Slugger was drinking alcohol and will be referred for the charge of felony Substantial Battery by Badger Police Department after he was witnessed to hit Sandra Williams with his open right hand to Sandra William’s left side of her face and then grab Sandra Williams by the back of her head and slamming her head onto the bar counter where she was seated. Sandra Williams was observed having a visible red mark on her face and a laceration on her forehead that later required eight stitches. On 11/08/2017, the agent received a police report from Badger Police Department Officer Konkol (Case number 1-98-00456723) stating Officer Konkol and Officer Sandner were dispatched to the K&D Tap (223 W. main Street, Badger WI) where they made contact with Joe Slugger.
• Per NYPD arrest report, MOS Harris and Denizard were responding to a 911 for robbery and when they arrived complaining witness identified P as one of the perpetrator. MOS Harris and Denizard identified themselves as officer and P took off running. MOS Harris and Denizard chased P and engaged in a struggle. P punched MOS Denizard causing MOS Denizard to fall to the ground and injury his knee. MOS Denizard was unable to walk on his knee.
On Wednesday 10-14-15 at approximately 1700 hours I Officer Hildebrand and Sgt. Hard were advised by dispatch that Debra Vanblaricom was at the station asking to talk to an officer. Debra wished to report an assault on her granddaughter. We met with Debra in the Police Department to speak with her. Debra stated that she feared for her granddaughter 's safety.

Taser International, Inc. (defendant) Facts: Darryl Turner, a seventeen-year-old grocery store bagger, is the deceased in this wrongful death case. Turner was fired and refused to leave the store’s premises peacefully, the police were then called. Police officers used a Taser twice on Darryl while taking him into custody, after collapsing he was unable to be revived. Tammy Lou Fontenot was charged with administering Turner’s estate.
12- 19- 2014 somewhere around 2400 and 0600 hours I Det. Scott Demeester was dispatched to 1710 Madrona Street. I was called in to investigate a homicide case. The victim was discovered tucked under a sheet in a vacant home in Atlanta Georgia.
The case to be presented involves the details surrounding the death of Mrs. Ursuline Alfred while under the care of the Hollywood Medical Center, now named Memorial Regional Hospital South, on May 18, 1996. Mrs. Ursuline was a forty-five year old female believed to have suffered a seizure in her home at approximately 1930. She was found on the floor by her husband, after shouting “My head”, in a coma-like state. Mr. Alfred activated EMS and the patient was transported to HMC immediately. While in transport, Mrs. Alfred recovered and EMS recorded “near normal” vitals at 0800 per the District Court of Appeals of the State of Florida (2012).
On March 7, 1998, 55 year old Mrs. Jadine Russell and her daughter were involved in a minor automobile accident. While on the side of the road assessing the damage, Jadine, her daughter, and two police officers, were taken by surprise, when alleged drunk driver, Keith Cook, lost control of his pickup hitting Jadine’s car. The vehicles then plowed into the four victims as they stood there. “All were hurt, but Jadine suffered the most severe injury when the car pinned her against a fence and ruptured her spleen, causing massive internal bleeding and leaking blood into her abdomen” (Baxtrom). Jadine, in need of serious medical attention, refused a blood transfusion.
On 05-22-17 at approximately 1906 hours, I was dispatched to Del Mar Boulevard and Edmondson Alley regarding a possible suicidal individual. Witness #2 Jaime Sanchez Huerta advised dispatch that Suspect Juan Hernandez-Trinidad was talking about killing himself, NFD. Huerta was reportedly following Hernandez eastbound on Del Mar Boulevard to Arroyo Parkway. Upon my arrival, I was flagged down by Huerta who was standing with Hernandez on the north sidewalk of Del Mar Boulevard east of Arroyo Parkway. I
Before I even step foot in the door I could hear sobbing and the distress in a woman 's voice. I carried in the necessary equipment, the cops were actively doing CPR as the advanced life support provider (ALS) was looking for IV access. As EMS personnel we took over for the cops, we placed a LUCAS device on him, it 's a device that does compressions for you. Then, we continued ventilating the patient and placed him on a backboard in preparation to transport and move the patient.
On 11-14-2015 at about 2106 hours I was dispatched to a domestic violence report at the Emerald Point Condos at 31900 104th AVE SE; H102, Auburn/King/Wa. Enroute dispatch advised the reporting party/victim, Lizbeth Nunez Santana was calling to report that she was assaulted by her husband. The incident occurred about two hours prior and the husband was no longer at the residence. I arrived at the address and contacted Lizbeth and her cousin, Maribel Chavez. Throughout my conversation with Lizbeth it was clear that she was very upset, she had to stop speaking several times as she was crying and upset with the situation.
On 02/08/2017, I, William Van Scoter, was working as a police officer for the Wichita State University Police Department, in Wichita, Sedgwick County, Kansas. At approximately 1520 hours I was told to interview a woman who was to believed to have been sexually assaulted. I arrived at the University Police Department's lobby and was met by a female and two males. The female was later identified as Terry Giang. Giang was going to be interview by Detective Jeff Rider
