Abstract Electronic Medical Records (EHR), technology has advanced and so has medicine. Treatments for many illnesses as easier to diagnose and treat, and documenting in a patient’s medical chart has become much easier too. Electronic health records have been integrated in many medical facilities, making it easier to and quicker to complete. In the past when a primary physician would refer a patient to consult with a specialist. It would take several days before the specialist would have the consultation which was needed. Now by the time the referral is generated and appointment is scheduled patient is already walking out with the copy of the referral, copy of the consultation of the visit, and appointment already scheduled. With electronic health records patient’s data is available at a click of a key to any authorized users. Medical staff are able to access medical and treatment histories of patients from a system which is designed to go beyond clinical data collection in any medical facility. Electronic health records contain a patient’s diagnosis, current and non-current medications, medical history, immunization dates, treatment plans, radiology in images, allergies and laboratory test results. Electronic health records allow physicians evidence based tools which are used to make decisions in regards to patients care or method of treatment. Make straightforward and promptness of documentation can be documented by another individual other than the clinician. With electronic health records it allows for a systematic research the point of care thought out a routine practice. It can easily be entered by the patient themselves, a family member or even a care team then information can be customized reports. Depending on the facility or organization, many will have computers installed in every room so when the physician walks in to consult the patient the physician is able to log in and document. After the consult then the nurse will come in but before she can even do her documentation physician must be logged off. Once the nurse is logged in she is able to schedule the patient for their returning appointment and given patient a copy of their visit summary and appointments which have been scheduled. That is an example of one facility in other medical facilities a computer is assigned to each staff member, this consist of a docking station located in their workstations, it includes a laptop which can be removed and taken to the rooms to allow for documentation and updating of medical history and medication reconciliation. This option is better than the first one only problem is when there is no electricity or internet one is left with nothing. The positive of having electronic medical records is that everything is at a click of a key and all medical history is available. This includes lab results, MRI, CT results, consultations from specialists, and referral history.
