The proposed program will address the needs of minorities 65 and older at the time the program is implemented. Those taking part in the program would not loose any of the health benefits they are receiving from Medicaid under the current SSI health benefits insurance program. As well the person or persons must prove that they cannot afford to reside in a traditional long term care facility, such as a nursing home or assisted living, without incurring out of pocket cost. To be eligible, the individuals must be African American, U.S. citizen, reside in the state of Mississippi, and have a median $650 SSI income. As well they are not required to liquidate there asset’s and must reapply every year to reevaluate their eligibility.
Management of Care Case Study Josepha is working on a medical surgical unit with three other RNs and one LPN. There is also a male and a female patient care tech. Josepha has been a nurse for four months, and after completing two months of orientation she takes a full assignment as a registered nurse. Josepha feels that the assignments she receives are not always fair, as she tends to get the most challenging clients.
Mildred Pasek, my friend and colleague died on August 8th 2017 following an anterior approached back surgery on July 28th at the New England Baptist Hospital. Before you read on, my goal is not to criticize the orthopedic or vascular surgeon’s professionalism, immense skill or personal care of Mildred as these cases affects all providers on a deep emotional and professional level. My concern is for the post-operative care of patients, like Mildred, who have comorbidities, are not necessarily in the ideal condition going into surgery, or at a critical time can advocate for themselves. As well as the debilitating arthritis, she had hypertension required three antihypertensive medications to control. Those meds were held pre surgery, and never resumed.

As when growing up the children try to cope with sudden changes, as from going to primary to secondary school. Not only will this affect them, but their friendship groups will also change too. The most common type of transitions the children go through are starting or moving school, illness, puberty, bereavement and parents splitting up.

School-reentry is a major issue for children who have been hospitalized for an extended period of time or who have had intensive medical treatments, such as patients in a major pediatric cancer treatment center. A hospital-school transition program for children being discharged from the hospital is beneficial for both patients/families and the school systems. To create an effective hospital-school transition program, it is important that a CCLS, the hospital teacher, social workers, doctors, therapists, school staff, the patient, and the family all work together as a cohesive unit. When discussing school-reentry, the CCLS, social worker, and the hospital teacher should provide information to the patient and his or her family on 504 and IEP

Users of long term care vary in age including young as well as the elderly. Many of these people need help with daily living assistance due to a disability or disease. The elderly may have more than one reason to need assistance. They could suffer from physical disabilities and illness, and have dementia or Alzheimer’s which affects their mental capabilities.
Upon arriving to the unit this morning, I quickly realized today was going to be a chaotic day with the current patient census, and all of the new admissions. I was able to assist the night charge nurse with today’s assignments, while she helped with the code, and the day began. I informed my team that today was going to be a long day, and encouraged them to use each other and myself for help. I recommended they taking a few minutes to coordinate their work after receiving report. At 0745, when Jane informs me that the patient in 408 has fallen, I am quick to get into the room and do an assessment again.
When undergoing transitions there can be a range of effects that children experience and can be observed. Effects which can be seen can be either short or long term. Different measures may be put in place to ensure that each child undergoing a transition is fully support and able to successfully get through the period of change. It is perhaps a common misconception by adults that children are quick to adapt and will therefore not be affected by a transition but this not always the case. Most children handle transitions incredibly well
The circulating nurse also initiated the time-out. During the time-out, the circulating nurse said the patient’s name, the surgery that the patient was getting, and the limb in which the surgery was being performed on. The other health care professionals agreed that it was the right patient, right site, and right procedure. Throughout the surgery the circulating nurse continued to ensure the safety of the patient by watching the surgical staff and making sure that the sterile field was not contaminated. This nurse’s role also included gathering materials for the surgeon, throwing away trash, and keeping the environment comfortable for the staff.
