Upon review of the sepsis collaborative data, SOH leaders identified a correlation between transitions of care between SOH and SNFs and readmissions that should have been avoided. Consequently, the leadership team formed an alliance with nine SNFs that serve the SOH patient population. This alliance served as a resource to develop effective communication between settings to determine how to better meet the needs of the patient, in an effort to avoid a readmission.
