Treatment Plan
Following the initial assessment, the counselor works with Cheryl to create a mutually created and agreed upon treatment plan. The treatment plan consists of specific goals that are to be accomplished by the end of her 30-day treatment and specific objectives that will aid in working towards the successful completion of those goals. Since Cheryl has previously completed treatment Cheryl and the counselor were able to critique her last treatment plan and create a treatment plan that would fill the gaps of what she was missing her last time in treatment.

Although they are supportive and helping through the process of getting treatment she reports that her parents have verbalized that she has to earn their trust back. The treatment plan indicates that Cheryl’s second goal is to increase her family’s faith and trust and rebuild their relationship. Cheryl and the counselor decided that her objective for that goal would be to attend a family process group once a week for the next 30 days of treatment. The family process group will allow them to openly communicate with each other and to learn more about the process of recovery together. She also received weekly counseling and case management for her depression and anxiety. As a part of MHMR program requirements all counseling she received followed the “Seeking Safety Curriculum” which is based on trauma informed care.
The intervention chosen was empowering to the client because she was able to take control of her recovery and sobriety. Cheryl was able to build up her confidence as it relates to her ability to maintain sobriety. The intervention had many strengths as it pertains to Cheryl’s treatment. Cheryl completed treatment feeling like she had accomplished her goals and objectives. She reports that she feels as though she made a lot of progress as far as rebuilding her relationship with her family and that she learned some new coping skills that she believes will be a great asset to her relapse prevention
