INTRODUCTION This is the case study of Ms Lynette Maree Young, 46 years old women who died on 29 April 2012 because of shock following Interferon alpha treatment. During the treatment of her in the hospital, failure to early diagnosis of pericarditis and failure to prompt management of side effects of interferon therapy led the situation for death of Ms. Young. The main reason behind the death of Ms Young has happened due to the lack of assessment and inappropriate care provided by the health team including doctors and nurses. All the incident and scenario of this case study clearly shows the lack of care and lack of attention towards patient’s condition by the doctors and nurses. According to the case study, when the Ms Young had sent to the Dubbo base hospital for further treatment she was triaged as category 2 patient which means she needs an hourly assessment of her vital signs. But the nurses has taken her observations twice only at 1615 and 1915 on 28 April, 2012 on Emergency Department. Even the Dr Lang did not record the finding of Ms Young’s ECG, chest x-ray and echocardiogram. Followed with the complained of chest pain front and back. The Nurse Mulavana who was the in charge of G ward on that day could not provide the evidence of any telemetry monitoring of Ms Young. It is clearly evident that Nurse Mulavana failed to provide the accountability of the patient. Furthermore, Nurse Skinner who was looking after the Ms. Young on the evening of 28 April, there were not any record of her vital sign, fluid administration and urine output. “Any failure of documentation that is needed as part of direct care has the potential to result in harm to the patient” (Atkins, Britton & de Lacey, 2014). AUTONOMY: According to Mcdonald & Then (2014) “It is not just providing information about the risks and benefits associated with proposed care and treatment, and the skills and expertise of the midwife or nurse providing care that recognises an individual’s autonomy.” The moral duty of nurse is to provide an information to the client and respect the choice of their decision.
