In order so that there could be patient centered care , the patient has to be the main priority and there has to be certain assessments that should be conducted. The nurse also has to plan accordingly to promote safety. When itcomes to looking at the case manager and medical record. The strenght for the Gardner family would be that they have a strong family support, the daughter of the patient is very involved. Mrs Gardner's daughter always visits the patient and making sure that the patient is stable. Mrs Gardner is also willing to cooperate with the medication administration and also participate in physical therapy session. There are many ways that the nurse can work with other interdisciplinary team, for instance the primary care doctor, the nurse can work with them by providing them with detailed report about Mrs Gardner. The nurse can also share a full report with the home health nurse about the patient, the nurse can also work closely with the physical therapist to help the patient mobilize better. The nurse can also work with the occupational therapist so that the patient could improve with their daily task. Overall, there must be a team effort when taking care of a patient especially a rehab patient, so that they can get the proper care.
