Supplies:Catheter Kit, Sterile Gloves, Wall gloves, Hand sanitizer, peri wipes,leg strapKNOCK ON THE DOOR AND ENTER PATIENT’S ROOMHello Ms. Patel, My name is ___________ I am going to be your student nurse today. (PULL CURTAIN, WASH HANDS)Can you tell me your Name and Date of Birth, please?(CHECK ID BRACELET)I am here to insert your indwelling urinary catheter. Is that okay?I see here that you are allergic to Iodine. Do you have any allergies to Latex or Tape?Do you mind if I lay your head back?I am going to raise your bed up to my working height as well.HAND SANITIZER AND PUT ON REGULAR GLOVESI’m going to fold back the covers and start with your Peri Care; I am going to go ahead and expose you and position your legs (LEGS SPREAD AND KNEES BENT)This may seem a bit awkward, but is this comfortable for you? I’m going to take a few wipes (3) and clean your Peri area. It might be a little cold.PERFORM PERI CARE FROM FRONT TO BACKTAKE OFF GLOVES, USE HAND SANITIZER, AND OPEN THE CATH KIT.OPEN KIT BAG AND SIT OUT KIT (STILL USING 8-12 INCH BORDER AWAY FROM THE PATIENT)I’m going to place this between your legs.Hold your legs really still for me. I’m going to open this kit.GRAB THE FOLEY KIT AND PLACE IT ON THE BED 8-12 INCHES AWAY FROM THE PATIENT AND ALLOW FOR A PROPER WORKSPACE.

BEGIN TO OPEN KIT AWAY FROM YOU FIRST TO AVOID REACHING OVER THE STERILE FIELD.STICK TO THE OUTER 1-INCH BORDER ON THE KIT DRAPE; YOU CAN TOUCH IT WITHOUT GLOVES AS IT IS NOT STERILE.REMOVE THE STERILE GLOVE. PLACE AND PLACE ON THE BEDSIDE TABLEREMOVE THE FIRST DRAPE BY ONLY TOUCHING THE 1-INCH NON-STERILE BORDER, AND PLACE IT ON THE BED WITH THE SHINY SIDE DOWN.GRAB 2NDDRAPE AND PLACE IT OVER THE VAGINA. THE DRAPE CAN BE FIXED ONCE STERILE GLOVES ARE ON.DO NOT FACE AWAY FROM THE STERILE FIELD, USE HAND SANITIZER, AND PUT ON STERILE GLOVESAsk the patient if they are ok.***PLACE ON THE DOMINANT HAND FIRST, THEN THE NON-DOMINANT HAND***!!!! HANDS MUST REMAIN ABOVE THE WAIST WHILE STERILE GLOVESARE ON!!!!Ask the patient if they are ok.FIX THE DRAPES IF NEEDED.GRAB THE TOP TRAY AND PLACE IT BESIDE THE BOX (CLOSEST TO THE VAGINA OR PENIS)REMOVE THE PLASTIC SLEEVE WHILE CONTROLLING THE CATHETER, MAKING SURE IT DOES NOT TOUCH THE OUTSIDE OF THE STERILE FIELD. ATTACH SYRINGE TO THE CATHETERREMOVE SWABS AND PLACE ON THE TRAYOPEN LUBE All right, Ms. Patel, I am ready to lubricate the catheter, and then I am going to cleanse the area a 2ndtime, just like I did with the Peri Care.LUBRICATE THE FIRST 3-4 INCHES OF THE CATHETERPLACE NON- THE DOMINANT HAND ON THE PATIENT. HAND IS NOW CONTAMINATED AND CANNOT COME IN CONTACT WITH ANYTHING STERILE. CLEANSE THE AREA.AVOID CROSSING THE STERILE FIELD TO DISPOSE OF USED SWABS.

Ask the patient if they are ok.Alright you are going to feel a little bit of pressure. I would like for you to bear down for me like you are going to pee. Deep Breath for me.Okay, I am seeing urine return.(FEMALE) I am going to insert the catheter another 1-2 inches before inflating the balloon.(MALE) I am going to advance to the bifurcation process and then inflate the balloon.HOLD THE CATHETER IN PLACE WITH THE DOMINANT HAND WHILE INFLATING THE BALLOON. You may feel a little pressure. Are you ok?PULL OUT AND CHECK TO MAKE SURE THE BALLOON IS INFLATED.REMOVE STERILE GLOVES, SANITIZE HANDS, PUT ON WALL GLOVES.I’m going to put a strap on your leg to secure the catheter there.Is that too tight?I’m noting your urine return. It looks pale yellow; there is no odor, around 300 ml, and no sediment. I’ll check it again in two hours.LOOKING AT THE AMOUNT, COLOR, AND CLARITY.
