This assessment will discuss the format and process of completing a person centred, holistic initial assessment and care plan for the fictional character ‘Rosie’. Person centred care is crucial to consider when assessing and treating a patient as you are making your care individual to the patient, ensuring that the patient’s needs, preferences and values are taken into consideration throughout their experience (NHS,2017). More so, treating the patient in a holistic manner, which may be demonstrated through considering behavioural, cognitive, social and spiritual factors, is also an example of good practice.

Equally to gather if she consented to having the assessment and if she needed any additional time or reasonable adjustments. This may be due to Rosie’s age and level of understanding; she may need more time to explain how she is feeling. The nurse also knew to base all work with Rosie around the Four Pillars of Ethics (Beauchamp and Childress, 2001), which would be demonstrated throughout different aspects of the assessment and care plan.
Additionally, the nurse took precautions to prepare themselves. According to The Royal College of
Nursing (2021) “All nursing staff have a duty of care to their patients, but they also have a duty of care to themselves which often gets overlooked”. Hence why in order to ensure safety the nurse followed the lone working policy and made sure to notify colleagues where they were visiting and when they were expected to be back in any event of something going wrong. Following on from this, the nurse then considered preparing the environment for instance, ensuring they would be in a private room to protect Rosie’s dignity.
