Nathaniel appeared somewhat slim. His additional concern was about not consuming adequate meal to sustain his health. He although recognized that eating normally it is vital to his well-being, as well as taking medications. “I feel shut off what is going on; my head is full of voices”. Although Nathaniel reported that he could take a shower, have a warm meal at the shelter, which is for the mentally ill homeless that is located nearby him, in a bad neighborhood where other homeless people are gathering on the sidewalk outside the gate. Case Conceptualization Nathaniel decided to seek outpatient psychotherapy because of his concerns regarding hearing voices, distress, and sadness. Taking medications can make him a more efficient musician. Nathaniel is a highly intelligent and talented, is cognitively impacted, and no obvious developmental problem. He has acceptable listening skill, some motivation for therapy, and is capable of self-reflection. In addition, it appears Nathaniel is suffering from identity issue, is having a hard time making a decision, and needs to determine his next course of action relatively soon. According to Beck (1960), the focus of behavior therapy is to allow the client to express his own thoughts about his experience. Stables decreasing re-hospitalizations, increase social skills, and decrease distress from symptoms such as voices and other hallucinations and delusions. Therefore, Behavior therapy can be beneficial for Nathaniel to create a better future by helping him make an effective decision and take action and control over his life. This will empower him to meet his goals and develop his identity successfully through the time-limited therapeutic
