Clinical Learning Experience My recent clinical learning experience was in Hebrew Home of Greater Washington at Rockville, Maryland, which is an elderly nursing home. It was a three Saturdays clinical experience in a sub-acute unit. The objective of this experience is to learn how to take care of the elderly patient who is transferred from the critical care to rehabilitate. I was assign to a unit for taking care of 6 patients. One of them was SB, a 75 years old African American female. SB has a long history of hypotension, hyperlipidemia and hypothyroidism. SB was calm and pleasant from my first impression. Because I was worrying about the actions I took may have big impact on the patient. For instance, the questions about her working history refreshed her great memories and opened a door of her internal strengths. I learned that as a health care provider, I have to take my time to do the interviews, identify the positive and negative factors to carefully provide options for managing the patient’s stress.
Since I have learned that a therapeutic relationship is essential for any caring procedures; I would first spend more time to know the patient and identify what are the patient’s major concerns and needs. It could be more effective if I knew SB’s fears and helplessness from the beginning. I also learned that by modifying negative self-concept can change one’s situation from a negative view to a positive view. I helped SB explored the positive dimensions and life stories in the past. SB became more optimistic about her life, and is willing to implement the coping mechanisms and social support. In summary, I learned how to establish a therapeutic relationship and identify the problems. I learned how to find resources and effective procedures in patient care. Taking care of multiple patients with a variety of illnesses is very difficult. In addition to knowledge and passion of caring, a nurse has to be a team player and a master of prioritizing and
