I want to provide effective counselling to the at least twelve or more patients per IPPE visit.

Objective:
• I will introduce myself to each patient I am counsel
• I will explain to the each patient about the name of medication, its side effects, how to administer, direction and the special precautions.
• At the end, I will make sure that patient I counsel understand the information I gave to him or her. At the end of year I will be more confident in providing counselling to the each patient.
Action Plan: Provide counselling to the patient and will ask pharmacist if I further need to add some points. Follow up with patient and regular basis that how the patient is doing on that medication …show more content…

GOAL 2
I want to spend at least an hour or more on insurance problems per IPPE visit.
Start date-End date: #August, 2015 - July, 2016#
Objective:
• I will participate more in resolving the third party disputes, I will take suggestions from pharmacist and technician regarding insurance issues.
• I will discuss with my pharmacist about cost savings program, co pay and other stuffs related to insurance.
• On my upcoming visit will talk about Medicare (Part A, B, C and D), on my next visit will talk about Medicaid, and then about blue cross and other insurances which can help me to gain a more information about insurances. I will be able to achieve this goal by end of the year, and will track my progress on each site visit.
Action plan: I will learn more about insurance program. I will observe technician while solving the insurance disputes. On each site visit will talk about one insurance program with my pharmacist.

• I will follow all the guidelines for regarding the safe and effective immunization; I will give at least five or more shots to the patients per IPPE visit.
• I will use the proper site of injection, proper gauge and length; I will load the syringe before I reach to the patient.
