D-The arrived late to her session, but had called this writer prior. Upon meeting with the patient, her demeanor appeared to be sadden, at which this writer addressed. According to the patient as she became emotional and a bit tearful, she is now experiencing homelessness. The patient says, " My boyfriend mother 's right, they had to leave. It 's some illegal eviction because there was no court paper and they were told to leave by the ending of the month." Furthermore, the patient says, " He and his mother moved into his mother, mother 's house and its section 8. I could not go." This writer explored alternatives such as family/friends; however, based on the patient comment, there 's no one she can turn too. YPP/AWOL: The patient missed group yesterday as she reports that she had overslept and was unable to dose. The patient AWOL twice in a week and signed a AWOL notice as the patient is aware about the risk factor of missing a dose which can lead to a relapse. The patient admits about relapsing and used heroin by IV- 5 bags. The patient reports that because she missed her dose twice, she began experiencing withdrawals such as sweats and cramps. This writer proceed to complete a dose change request upon the patient request to increase her dose by 5mgs. Other: This writer updated the patient about a recent conversation with her probation officer and informing the officer about the patient 's progress. The patient did not explore any mental health services as she currently need to resolve her homelessness situation first.
