On 11/4/15 Ms. Brittany Terry and her daughter Lundyn met with CM for an ILP. CM introduce herself as the client temporary CM assigned to her case. Housing: Client 's exit strategy is to move by 2/18/16. Client must maintain employment and submit pay stubs weekly .Client is expected to comply with savings and have saved enough for one month 's rent and security by 12/18/16, Client is to seek for housing and obtain housing by 1/18/16.Client was reminded she must meet with her Housing Specialist Mr. Brown on a bi-weekly basis, comply with all housing requirements and explore all available housing options. Social Service: Client must meet bi-weekly with Case Manager for ILP Review appointments.
Goal: Cm met with client for weekly follow up I: CM greeted client and followed up on last week’s session. CM reintegrated goal of the Harm Reduction program was to provided a referral for housing. Intern discussed plan of action to help manage the Client’s access to VA benefits and referral for VASH. Intern validated the progress the client is making and his interest in doing well.

G-CM met with the client to fill out a Skid Row Housing application, and give directions to the nearest social security office. I-CM helped client fill out a five page application for Skid Row Housing. CM gave the client the closest address to the social security office, because the application requires a copy of the client's social security card. CM also advised the client to continue attending different group meetings to help with his social skills and cope with his depression. CM continued to assess client's MH and medication.
Phase One (Sessions 1 through 3) • The session 1 and 2 consists of the assessment of the client’s clinical problems and background information. Questions relate to her clinical problems, including (a) the nature of her problems (depression and difficulty with making decisions), (b) reason of seeking psychotherapy, and (c) previous attempts to deal with the problems. For the background information, the client’s histories are assessed in the areas of intimate/family relationship, educational/vocational activities, past history of psychological treatment, and physical condition. Session 3 primarily consists on the further functional assessment around her coping skills (e.g., avoidance) in the areas of interpersonal relationships. Phase Two

As I progress in my career, I feel that it is important to develop a personal style and technique that clients can relate to. I am also interested in learning more about assessments and evaluations as they drive the rehabilitation plan. While I have assessed hundreds of clients, I realize that there is more to learn. Description: Prior to shadowing a physician at a community health clinic, I had the chance to observe one of the clinic’s physician’s assistants for a day. Victoria, the PA, met with both existing and new patients, which helped expose me to different aspects of her practice. Her appointments included assessing a chronic thyroid condition, following up on previous treatments, and a full physical. REFLECTION OF THE INTERVIEW 2.1 Description A component of the Effective Communication module, is an Inter-professional Education (IPE) enrichment activity and we were put into groups with the Pharmacy students. There were in total 2 Nursing students and 3 Pharmacy students. We were tasked to interview a healthcare professional. We interviewed Dr Edwin Lim from Tzu Chi Free Health Screening and Free Clinic (TCFC). He specialises in Family Medicine.
