SUMMARY Up to 150 words summarising the case presentation and outcome (this will be freely available online) We report a case of 38 year old female presented with right hypochondrium pain was found to have gallbladder dyskinesia and falciform ligament cyst, a very rare entity. This falciform ligament cyst was initially misinterpreted as simple liver cyst in an ultrasound scan done elsewhere. Cross sectional imaging helped to correctly diagnose the falciform ligament cyst and it is later confirmed with surgical and histopathological evidences. There was no sonographic evidence of cholelithiais or choledocholithiasis. HIDA scan revealed sluggish & delayed emptying by the distended gall bladder even after fatty meal with an ejection fraction of <14%. Contrast enhanced CT scan showed a benign appearing cyst of water density with imperceptible wall measuring 3 x 3 cm noted along the falciform ligament between the right and left lobe of the liver suggestive of falciform ligament cyst. No significant abnormality was seen in chest radiograph. Her complete blood count, renal and liver function parameters were found to be within normal limits. Final diagnosis of gall bladder dyskinesia and falciform ligament cyst was made. DIFFERENTIAL DIAGNOSIS If relevant TREATMENT If relevant The patient underwent laparoscopic cholecystectomy for chronic cholecystitis with complete excision of falciform ligament cyst. Surgical excision of the cyst was done with ease as there are no surrounding adhesions. Post-operative period was uneventful. OUTCOME AND FOLLOW-UP Histopathological examination confirmed the diagnosis of chronic cholecystitis and benign simple cyst of falciform ligament origin.
