According to DSM- 5 (2012) 5th, manic episode’s criteria: A. A distinct period of abnormally and persistently elevated, expansive, or irritable mood, lasting at least 1 week (or any duration if hospitalization is necessary) B. During the period of mood disturbance, three (or more) of the following symptoms have persisted (four if the mood is only irritable) and have been present to a significant degree and represent a noticeable change. • Inflated self-esteem or grandiosity • Decreased need for sleep (e.g., feels rested after only 3 hours of sleep) • More talkative than usual or pressure to keep talking • Flight of ideas or subjective experience that thoughts are racing • Distractibility (i.e., attention too easily drawn to unimportant or …show more content…

He has inflated self-esteem and grandiosity (he thinks everyone knows him, interrupts the therapist speech, attempts to control session, has arrogant style, brings a coffee machine without asking for permission). He reported that he watched his wife as she sleeping which might be a sign of decreased need of sleep. He is talkative but whether it is his normal or not is unknown. He can be distracted, and stands out at times in the sessions. He reported that while bombarding the madrasa, he just focused on his job and did not think of its results. He seems to be fully goal-directed and reckless about its consequence. On the last session he stated that he bought several items which is not very essential, and it suggests engaging in unrestrained buying sprees. In addition, he abruptly decided to leave the home and began relationship with another woman which might be sexual indiscretions. Currently, his symptoms are not as severe as manic. On the other hand, two things in Alex’s history points out the manic episode. First one is his description about the madrasa bombarding (fully goal oriented, and does not care about result). Second one, right after the bombarding, he attempted to run twenty-two miles without considering his physician friend’s warning and as a result of it, he had a hearth attack. First, his general behavioral pattern obviously points out the narcissistic personality features. He shows pervasive pattern of grandiosity, lack of empathy, has a grandiose sense of importance, and believes that he is special and unique. However, he does not violate the right of others all the time. When the therapist did not accept the coffee machine, in spite of his moderate resistant, he corresponded with the therapist’s wish and attempted to find another way. In addition, he reported that at the beginning, he had a hard time to embracing his son’s introverted characteristic, but now he admires his son independent and humble personality. Which is not compatible with narcissistic personality disorder, therefore it is ruled
