Generally, both musculoskeletal rehabilitation and neurological rehabilitation aim at the same goal, i.e. to prevent complications, minimize impairments, and maximize function and the well-being of the patients. Therefore, the application of musculoskeletal rehabilitation and neurological rehabilitation would be quite similar, but of course there are some differences as well so as to make each of them unique in the rehabilitation field. In terms of the target patients, they are different as musculoskeletal rehabilitation is concerning to the muscles, bones, tendons, joints, nerves, and cartilage. Whereas concepts used in neurological rehabilitation are more likely stress on the re-education of the functional ability and movement of the patients as most of them have damage or dysfunction of the central nervous system (CNS). The basic idea of IMT is to strengthen the inspiratory and expiratory muscle of the patients with COPD so as to improve the problem of shortness of breath (SOB). An meta-analysis on the effectiveness of IMT has been proven, showing that IMT plus exercise program increases inspiratory strength and endurance and a significant effect for decrease dyspnea at rest or during exercise (Lotte et al., 2002). It can be achieved by the assistance with breathing devices like breather, threshold IMT and DHD muscle trainer, they provide resistance that train the respiratory muscles through conditioning.
