Oral health is an important part of the general health and important factor that impacts an individual quality of life (DDF, 2005). Individuals with an increased concern about their facial appearance have lower quality of life standards and tend to isolate themselves from the society and any social interaction ( Oral health in America : a report of general surgery ,2000) .the result the consequence oral health may have huge impact on physical , social , and psychological wellbeing of a person (Locker et al,2004).

Conceptual model :
Locker model of oral health is based on WHO (Locker, 1988) the model he proposed was to cover different areas which include:
 Physical disability.
 Handicap.
 Physical pain.
 Social disability.
 Psychological disability. 2. Statement about impact:
Open ended questions were asked to 64 dental patients to evaluate the impact of oral disease on them. This interview took place in Adelaide Australia ,64 patients who participated in the interview were adult patients. The patient’s interview resulted in 535 statements which were reevaluated and the result was 46 unique statements .The statements were divided into 7 conceptual dimensions and three statements were added to cover the handicap dimension.
3.Structure of questionnaire :
OHIP questions evaluated on five point Lickert scale, for example when the patient choose there is only the following choices (S.Adulyanon & Sheiham):
 Occasionally.
 Hardly ever /never.
 Very often.
 Fairly often.
4. Scoring :
According to (S.Adulyanon & Sheiham) the scores is coded from 0 to 4 , after the patients answer the questions the codes will be summed up the result will indicate the impact of the problem on individual life.
