My patient is an 89 year old male; the patient will be referenced as WP. WP was admitted to Lutheran 3 East for a primary diagnosis of pneumonia. His health history consists of COPD, acute respiratory failure, chronic kidney disease, coronary artery disease, vascular dementia without behavioral disturbance, CAD, carotid artery disease, and hyperlipidemia. My client also has a permanent cardiac pacemaker and use hearing aids. He’s allergic to oxycodone and Vicodin. His current health is with pneumonia being his primary problem, he is using oxygen via nasal cannula, wheezing present in lungs, alert, and HOH. I first started my day my visiting with my patient in the morning. I during my time with the patient I took his vitals and assessed his skin. When the tech came in, I helped move my client from his bed to the chair. Once he was sitting up on the bed I assessed his back for any lesions and listened to his long sounds that presented with a lot of wheezing. He had IV on both arms. In the room he had a walker, glasses, urinal, and a nebulizer device. My patient uses hearing aid in both ears. When I first went in to meet him, a son was visiting him. He was coughing at times and wheezes when he would breathe. As time went on, I was able to notice his weakness in his legs and hands. His weakness helped me understand the need of assistance of two when helping him move. As my patient had difficulty hearing, I made sure to place his hearing aids correctly each time I saw them out of place. Though when he was eating his breakfast, he wasn’t in need of assistance, but he needed assistance for taking his medications. I used applesauce and placed one pill at a time on it and placed the spoon in his mouth; as I did that I made sure that he was swallowing the pills and offering water when he
