Many psychiatric patients are hospitalized in psychiatric centers, according to the prediction of World Health Organization, their numbers will increase by 15 percent till 2020 [1]. The health and well-being of patients is health care providers’ first priority, and they face challenges to provide quality and safe care in changing conditions, promoting the quality and safety is a constant challenge for them [2-4]. Safety is one of the important areas of health care quality (safe, patient-centered, effective and based on evidence, efficient and fair) [3]. The inductive manifest content analysis process is used to summarize the studies that aim to identify and describe the phenomena. First, interviews were transcribed word by word and studied several times to get an overall sense, then the text was divided into meaning units that were condensed. The condensed meaning units were compared, discussed and labelled with a codes. Then the various codes were compared based on differences and similarities and sorted into thirty four subcategories. Eight categories created from the similarities and differences and seen between the sub categories. Categories were formulated into three main categories (Table 1). In the last phase of the analyzing process, the latent theme was created from a deeper interpretation of the underlying meaning of the subcategories and categories. The data was managed using software
