Rebecca's Evaluation: A Case Study

Rebecca arrived on time for her evaluation. She presented as pleasant, and attempted to engage the examiner in conversation. She readily responded to the examiner’s questions prior to the beginning of the evaluation, which allowed for some rapport building. She exhibited good work habits and appeared interested in the testing process, as she was attentive to the test stimuli and followed the examiner’s instructions. In addition, several times throughout the evaluation, Rebecca stated “this is fun, I like this stuff”, referring to the test materials. She also seemed interested, as she frequently inquired about her scores on subtests. Rebecca was cooperative and engaged for the duration of the evaluation. She presented with adequate hygiene, was well groomed, and dressed appropriately for the setting. She did walk with a cane, and breathed through oxygen tubes throughout the evaluation. Affect Rebecca presented with euthymic mood and congruent affect. However, at times, she presented with liability of mood, as she began to cry after not being able to complete block design, and during a subtest on the MAS. In addition, while completing the PAI, Rebecca stated that “I am very depressed”. However, throughout the evaluation, she was observed laughing, making jokes, and statements like “this is fun”. Overall, she was pleasant and appeared to be in good spirits for the duration of the evaluation, and her mood was consistent throughout the day. Unusual Thoughts/Behaviors During the Similarities Subtest on the WAIS-IV, Rebecca stated “I’m going to have a mental breakdown”. During the Lateral Dominance subtest on the HRB-A, after being asked to demonstrate how to use a knife, Rebecca stated, “oh, I was going to show you the cuts on my wrist”.
