The reentry model was developed by the National Institute of Corrections to help the state correctional departments develop their own transition programs from prison to the community. A prison case manager develops a transitional accountability plan that serves as an assessment of what each offender needs to accomplish prior to their release date and in the community. They are generally transferred to a transitional housing unit when they are close to their release date so they can work on goals for once they are back out into society. There can be many challenges when reintegrating prisoners into society, some of these issues can overlap and multiply when various aspects of their lives are mismanaged, neglected, or mistreated. These things can include affordable and stable housing, secure employment that pays a sufficient income, making sound decisions, and having reliable public transportation. They also need to have and choose to have supportive relationships within their lives and that can come from friends, family, kids, and even those they are in a relationship with. They need to connect with people who do not have criminal attitudes and behaviors, and need to work towards regaining legal and physical custody of their children if it is applicable. These stable relationships will give them someone to turn to or lean on when they are having a difficult time or if they are struggling with choices and decisions about their life. They also need to work on connecting to social services and community agencies because they can lend them help when it may be needed. They can help with sobriety, recovery from drugs or alcohol, parole obligations, and can loan insight into problems and understanding behaviors.
