The staff nurses and patient care technicians (PCT) in 6 East were not getting the sufficient education reinforcement regarding fall prevention. Consequently, this has created a knowledge gap among staff members regarding fall prevention strategies. The knowledge gap in fall prevention has led to an increase of staff non-compliance with the policy and the trending increase of fall rates in the unit during the 1st and 2nd quarter of 2015. The plan to mitigate this problem was developed through the collaboration with the nursing leadership on the provision of staff education on fall prevention. The novel, The Souls of Black Folk, written by W.E.B. Du Bois, illustrates the racial discrimination that was present amongst the whites and the African Americans following the Civil War. The readers follow Du Bois and the African Americans through their daily hardships of being people of color. Being a man of color himself, Du Bois, struggles existing amongst the two worlds; the world of white superiority and the world of cohesion as African Americans. Du Bois’ availability to these two worlds is known as “double-consciousness”, where he sees two sides of an idea. This ability can be both a blessing and a curse, and according to Du Bois he agrees, “He simply wishes to make it possible for a man to be both a Negro and an American, without being cursed and spit upon by his fellows, without having the doors of Opportunity closed roughly in his face” (Du Bois 3).
This protocol recommends a fall-risk assessment and implementation of a fall prevention plan for every resident, as well as an incident report for every fall. The other part of the agreement consisted of things that the staff can do to prevent falls and some examples included helping the client when they are weak, using a bed alarm for the client, keeping the room clutter free, checking up on the client hourly. According to Hoke & Zekany (2022), “after implementing the fall prevention agreement, nurse leaders reported a decrease in falls and falls with injury, which has been sustained for the past 6 years...” (p. 638). Utilizing a fall prevention agreement ensured that the staff and the clients were educated on fall prevention and that falls were reduced. Solares and Calero (2022) discussed the importance of how having the client involved as an active part of their care can improve care for the client.
Problem Identification Getting out of bed is one of the dangerous things that the elderly patients do when they are admitted in the hospital. Study conducted by Ambrose, Paul & Hausdorff, (2013) on patient falls reveals that a majority of falls in the elderly patients occur between 0700 and 1900, especially when they are getting out of bed to use the rest room. The cause of their falls is mainly due to unsteady gait, memory loss, confusion that comes with age. Memory loss and vision problems which occurs during old age in the elderly patients puts them at risk for falls. Other factors that can lead to falls are; Presence of throw rugs, psychotropic medications, lack of Vitamin D, and weakness of the lower extremities.
It also provided the use of critical thinking and clinical judgment on how to prevent falls, support, and be accountable for a client professionally. The practical knowledge I have learned helped me become aware of assessing and assisting a client. As a nurse, our job is to provide “safe, compassionate, competent and ethical care” (p.8) and collaborate as an interprofessional team to deliver safe care and prevent risks from happening while offering quality nursing care (CNA, 2017). I will always provide the professional care under the code of ethics to promote health and wellness for an older adult and prevent risks from happening. As well as following the plan of care, use communication strategies, be aware, acknowledge, and accommodate individuals with different diseases such as with dementia, to promote fall prevention strategies (RNAO, 2017).
Informative:Fall Many people in the Fall tend to lose weight or gain weight some more than they wanted to, so they start thinking of ways they can lose all that weight they have gained, but you can also gain weight by doing different this. Exercising will help you gain and lose weight, just depends on what you do, for example say you are loving how the weather is outside so you decide to go for a jog that is a type of exercise. Another way is weightlifting, that can help you gain weight because you are gaining muscle also, so my job is to help us find some different ways to stay healthy this Fall. One way to start is taking advantage of the weather.
I will also discuss on how this clinical situation could be done differently. Clinical scenario I was posted to a medical ward in National University Hospital for my clinical posting. There is a particular cubicle allocated for patients with very high risk of fall called the “Green eye cubicle “. Patients in that cubicle are usually confused or not compliant to fall precaution.
As all know, the incidence of patient falls will be the big thing in every health care centre. I also worry if Madam Y experienced any complications, I might not be able to forgive myself. This critical incident made me feel sad and disappointed in myself. After this incident, I started to blame myself for the fall and this affected my nursing practice until the end of my shift. I still being uncomfortable and not confident on that day while performing my nursing skills and felt sad throughout the day.
