It is not sufficient to rely on the physician order to evaluate and treat the patient, to substantiate the services rendered and billed. The 2015 OIG Workplan maintains their objective to reduce the number of false claims submitted by nursing homes for services rendered through rehabilitative clinicians. The licensed clinician has the burden to validate through the evaluation, plan of care and ongoing assessment of the patient why the technical skills provided by the discipline delivering care, is essential to the patient achieving the goals set forth in the care plan. Completion of documentation within the timeframes required, such as recording 30 day summaries at a minimum for Medicare-A beneficiaries and or daily encounter notes when treating
I think Julie's behavior is unprofessional and unethical due to the fact being medical assistant only and not a doctor. For that reason any medical staff including Julie face ethical issues in a medical office on the daily basis. There are some guidelines Julie should follow while working with any patients: -Provide services with full respect for human dignity -“Aspire to render great service” to patients -“Dedicated to the care and well being of patients” -Respect confidential information unless required or authorized to disclose in a legal manner -Seek to continually improve knowledge and skills -Participate in services aimed at improving the well-being of the community and so on. Medical assistant should not diagnose and discuss a patient's
The OT will look at how things may have changed for Taylor after a TBI and SCI and then recommends different ways to do tasks or provide modified equipment to support him to be independent as much as possible. This includes recognising levels of care and help needed to complete tasks. PT should conduct both the physical assessment and planning of physical therapy. They can assess and promotes mobility, movement, muscle strength, coordination, balance and stamina. PT can also provide information on walking aids, develop a fitness/mobility plan and provide training to Taylor’s family/caregiver.
Dilemmas are situations in which a difficult choice has to be made between two different options which can be taken. The Mental Capacity Act 2005 outlines a legal framework for making decisions on behalf of those who lack the mental capacity to make specific decisions on their own. All those working with and caring for an adult who may lack capacity to make particular decisions must comply with this Act when making decisions or acting for the individual, when the individual lacks the capacity to make that decision for themselves. This applies to life changing matters as well as every day matters. The Act firstly looks at whether an adult over the age of 16 has the legal capacity to make decisions for themselves, unless they show signs of
Depending on the recommendations that his primary physican has suggested the therapist can take that information with the results from the assessment tools to provide additional referrals and resources available to him. Additionally,
Imagine if a sixteen-year-old boy turns into a father as an alternative? Night is a memoir of Elie Wiesel’s stories as a young boy that traumatizes him for his existence. The appearance of Nazis in Transylvania deports him and his family to a concentration camp. In 1955, in Mississippi, the novel tells the authentic story of the kidnapping and homicide of an African-American teenage boy, Emmett Till. It depicts the trial of his murderers from the perspective of Emmett Till's white friend, Hiram Hillburn.
A therapist may do this once again in order to continue to receive reimbursement from Medicare for their services in order to generate more revenue for the clinic. Prior to the Jimmo versus Sebelius case in 2013, Medicare would deny coverage to patients receiving maintenance care for their condition due to their inability to demonstrate improvement from the skilled therapy services that they received (“Jimmo v Sebelius Settlement Agreement Factsheet,” 2013). However after the Jimmo versus Sebelius case, the Centers for Medicare & Medicaid Services revised portions of the manual which now states that coverage “does not turn on the presence or absence of a beneficiary’s potential for improvement, but rather on the beneficiary’s need for skilled care” (“CMS Manual System,” 2014). The correct action for the therapist to take in order to be in compliance with the rules is to provide adequate documentation as to why the patient would need and benefit from maintenance care services in order to prevent or slow the deterioration of their condition. An alternative would be to discharge the patient if they no longer require skilled and medically necessary services (“Jimmo v Sebelius Settlement Agreement Factsheet,” 2013) (“CMS Manual System,”
The article describes correctional nursing as one of the hardest jobs to keep values in such, ethically and legally in a sense of what it means to be a nurse. The reason their job is so hard is they have to set aside their feelings and view on things they don’t necessarily agree with. Whereas, nurses in a hospital setting or clinic don’t have to make or choice if they want to help a patient, they just do because it’s the duty of the nurse under the American Nurse Association. The correctional nurse has to be able to deal with incarcerated inmates every day and on a daily basis. They have to be trustworthy of the inmates who may have done something against their beliefs and religion.
Today's health care system is difficult to understand. It has undergone dramatic changes over the years. There are many changes that shift the movement from "an indemnity plan to a managed care system. " Not only has the U.S. health care system undergone dramatic changes, but as well continues to evolve to a rapid pace (Conklin, 2002).

D-The patient arrived on time for her session and informed this writer that she has decided to remain with the clinic as she learned on her own that no detox facility will accept her because she is testing negative and currently on methadone. The patient further mentioned that she is questioning as to whether or not her sister and her mother would help her as they said they would; however, the patient had a moment and looked back when her family did not help her as she struggled with her children. As a statutory adult social worker, I am responding to a service user’s self-referral regarding support for herself and husband, Ian, on hospital advice after Ian suffered a couple of falls. Jackie, 69, is a carer for Ian, 66, who has increasing needs due to dementia. My duty is to conduct a needs assessment for Ian and a carer assessment for Jackie to identify the impact upon their wellbeing, including the effect on relationships with others (Care Act, 2015: c23:p1:s1). The visit’s objective is to obtain better outcomes and maintain independence to enable Ian to remain at home for longer.
An ethical dilemma happens when two or more ethical principles conflict with one another. Ethical dilemmas are problematic situations in which it is not clear which choice will be the right one. The CP is stuck as to what to do next because there is not just one outcome that will satisfy the ethical principles as stated in the Singapore Association of Social Workers (SASW) Code of Ethics (Kirst-Ashman & Hull, 2012). According to the SASW (2017), the core values of social work are embedded in the ethical responsibilities, which are relevant to the professional activities of social workers.
As I progress in my career, I feel that it is important to develop a personal style and technique that clients can relate to. I am also interested in learning more about assessments and evaluations as they drive the rehabilitation plan. While I have assessed hundreds of clients, I realize that there is more to learn. For instance, the trend over the next five years will be an increase in applicants who receive state-plan home and community based services. The Final Rule issued by The Center for Medicare and Medicaid will require that individuals receive services in the most integrated settings.
I believe that humans being are born neutral, we are neither good or bad. With this being said, I believe that everyone has the chance of becoming decent person or an indicent one, and that we all choose our own destiny. However, I also believe that we do not just become a decent person overnight, it is a process, in which many mistakes have to be made in order to become who we really want ourselves to be. So, in my opinion the criminal justice system in the United States today should give those who committed a mistake a second chance, by letting them become who they really want to be and by giving them the tools and help they need to get there. I strongly believe that rehabilitation can make people better and reach their goal of becoming a
