Lately, I watched the documentary “RX the Quiet Revolution” by David Grubin. I found it a very interesting movie that introduced us to a different health care delivery system by lowering the cost and increasing the quality.
The movie discussed the challenges in the health care delivery system in America and the transformations that are being made to make the patient at the center of the care with a lower cost. Dr. Douglas Eby, the VP of medical services at Southcentral Foundation, started the movie with a good example of the rock, bird and target. It is part of nationwide program called Program of All-inclusive Care for the Elderly (PACE) funded by state and government. It allows seniors with chronic illnesses or disabilities to receive care while living at their home because the founders understood that people wanted to live at their own home while aging out of nursing homes. The organization has a team of healthcare professionals who meet to discuss changes in members’ physical and emotional health. Their foundation is very similar to the nursing home, which is an Accountable Care Organization (ACO), where groups of doctors, hospitals, and other health care providers, voluntarily come together to provide coordinated high-quality care to their Medicare patients. But seniors feel lonely there and get into depression and It is more likely for someone residing there to be sent to the ER and be hospitalized and then to be readmitted to hospital in the next 30 days compared to the On Lok where less people visits ER and stays in hospital. The nurse practitioner in On Lok talks about one of its member who participated in On Lock pain management class years ago after having stroke and some falls and since then how her health has improved without much medications.
