Mr. Gershon is a 33 year-old Caucasian male, single, has one child, and unemployed seeking treatment for his Substance Abuse issues. Mr. Gershon reported he has struggled with his addiction from 13-years-old and noted his addiction has progressed into his adulthood. The client reported as a result of his addiction he is unable to keep a steady employment, currently homeless and lacks good coping skills to maintain his sobriety. He reported he would like to gain the tools to stay clean.

Gershon shared he maintained average grades throughout. He denied he was offered any special classes for his learning. Per the client, while in high school he used drugs and was expelled because of his usage. Nevertheless, he noted he was given the opportunity to return to school and obtain his GED. However, he shared he was prescribed medications for issues related to his sleeping pattern. He noted he had suffered with sleep disturbances since he was a child, but denied he received any medical attention for this problem. Additionally, he reported he suffered from seizures due to withdrawals symptoms from xanax 7 years ago. Mr. Gershon also encountered several traumatic events in his life. He shared he has experienced childhood trauma when his father left home when he was 8yoa. He indicated it was difficult for him to understand why his father left. Mr. Gershon, further noted between the ages of 2-3yoa he was sexually molested by his adopted sister. He noted he remembers a little about the incident. The client also learned of the passing of his adopted sister, 2 months ago, from an overdose. He noted, although she was not his biological sister in some ways they were similar and could relate to one another.
