According to The Oxford English Dictionary, it gives a definition of plagiarism as: The action or practice of plagiarizing; the wrongful appropriation or purloining, and publication as one's own, of the ideas, or the expression of ideas (literary, artistic, musical, mechanical, etc.) of another. The origin of this word is from a Latin word “plagiarius” which means “kidnapper” in the early 17th century.

As the name mentions, unintentional plagiarism is done unintentionally. It is also called as accidental plagiarism. It is mainly caused by a poor paraphrasing (for example, changing some words without changing the sentence structure), a poor quotation (for example, putting quotation marks around a part of a quotation but not around all of it) and also a poor citation (for example, citing inaccurately).
