Cedar Rapids v. Garrett F. Garret F., was a quadriplegic who was ventilator-dependent due to his spinal column being severed in a severe motorcycle accident when he was 4 years old. During the school day, he required a personal attendant within hearing distance to see to his health care needs. He required urinary bladder catheterization, suctioning of his tracheostomy, observation for respiratory distress, and other assistance. He attended regular classes in a typical school program and was successful academically.
On 1/17/2016 SO EMT Perez was dispatched to HG-407. SO EMT Perez knocked and announced his presence at the door. SO EMT Perez was greeted at the door by the resident. The Resident, a Mrs. Elsie Cooperman answered the door and was activly bleeding from her face as she was trying to explain what had happened. SO EMT Perez immediatly began to controll the active bleeding and had Mrs. Elsie Cooperman sit down in a nearby chair while he tried to also calm the resident down so he could get some information about what happened.
Spoke to Vivian Ortiz (DOB 6/20/75) who stated that her juvenile son (16 years old), Elijah Perez (DOB 7/3/99), was being destructive and threw things around the house when she asked him to turn off the oven after he used it. Ortiz stated Perez had been diagnosed with intermittent explosive disorder and he had not been taking his medication or to his therapy. Ortiz also mentioned that she opened the case with DYFS to evaluate him and the case was still opened. Ortiz stated that Perez threw some baseball cards, laundry bin, trashes and made mess inside of the refrigerator. We weren’t able to speak with Perez as he walked out of the house prior to our arrival.
CCIB received an Incident Report from the LTCO Maged Mansour (213) 351-6433 regarding resident Maria Alcantara. The SOC 341 states that Maria disclosed to facility staff and her Day Program staff that residential facility staff member Gloria G hit her in her back. Once notified by of incident the facility staff notified the administrator Kendal Woods (323) 208-0133. According to the RP the incident is under investigation. The suspected abuser would be suspended pending
When they would take breaks during the night she would go and collect information. When she got the information she wrote it down. And
Clara began her newest occupation immediately, with the help of an instructor, who demonstrated the proper approach for several machines. As the weeks began to mold together, Clara 's knowledge increased as well as her ambition, her desire to learn as much as possible became unconcealable. During an examination, Mr. Samuel Parsons, noticed excessive heat orginating from a machine. To inspect the machine, he inserted his arm to lift the machine to investigate the main cause, and almost immediately his arm was enveloped in flames.17Mr. Parsons fled from the building in terrifying screams as the fire raged on, and in a matter of three hours, nothing was left behind.
On 3/29/2016 I spoke with Leahanne Garcia at the operation. Leahanne stated she has been employed at the operation since May 2015 as a Teacher assistant. Leahanne stated on Wednesday (3/23) that the children had just woken up from nap and Andi was leading circle time, while she was moving the block center shelf back to its position. She stated the children had started to get up from the rug for center time when she heard Andi yelling "get up, get up" and was yanking Isiah up by his arm. Andi then started telling him "you will listen to me" while pointing a finger in his face.
Officer Perez reports being dispatched to a disturbance at 11 Langton. Perez arrived on scene and met with Ann Mcelroy that stated she lost her phone and found that 2 kids had possession of the phone and was able to get the phone returned to her. Mcelroy stated she wanted to make a report of her phones information had been erased and was concerned of any personal information that was possibly had been taking prior to her getting her phone back. Perez had Mcelroy complete a witness statement form.
Amber Butz 219 E. 9th St. Northampton, Pa. 18067 (610) 844-8174 was advised of the identity of Investigator Sean P. Brennan and of the confidential nature and purpose of the interview, Butz, provided the following information: Butz explained that she started on Judge Tidd’s staff in September 2012 until December 2013. Butz is now working in the 911 Center in Northampton. Butz is the daughter of Brenda Anthony. Butz was starting to learn how to do the paperwork when Tidd would tell her he needed her to run and get him a soda at the gas station or run over and get him something either to eat. When Tidd would tell her Butz to do this or that she felt like she couldn’t say no to him.
On 6/19/16 at 2341 hours my partner, Officer Acosta #0044, and I were working patrol assigned to unit 3A22. We responded to a radio call of “shots fired” in the area of 101 W. Washington Bl. Prior to our arrival, Sergeant Gomez #3196 arrived on scene and advised that there was a female, victim of a possible gunshot in front of 101 W. Washington Blvd. Sgt Gomez requested the Pasadena Fire Department (PFD), Rescue Ambulance, (RA) to respond. PFD stated they would stage until code-4.
An face to face interview was conducted on 10/16/15 with Mary Ann Zimmer the nurse for Children's Arkansas Program at the operation. Ms Zimmer stated she was notified there had been an injury on the Wellness Trail and had went with to meet the class. Ms. Zimmer stated when she arrived at the Wellness Trail she was met by the class and one teacher was leading the class and the other was hold a child with a papertowel and ice pack applied to his chin. Ms. Zimmer stated she took the child and went inside to her office where she looked at it and determined the parent needed to pick the child up. Ms Zimmer stated the chin was spilt open approximetely 1/2 inch.
Special Populations Agency Interview On February 16th of 2016, an interview was conducted with Mrs. Sharon Mosley Walker. Mrs. Walker is a Commission Investigator for the State of Florida. She was a former Protection Investigator (PI) for Children and Family Service of Jacksonville, Florida. She also worked as a Probation and Correctional officer for the State of Florida.
