C: Mrs. Mack (MHS) reported Jaheim Brown display physical and verbal aggression. A: At 9:58 PM Ms. Smalls (on call case coordinator) was notified by Mrs. Mack that Jaheim started throwing things around the home, refusing to comply with requests and refuse to communicate with Mrs. Mack. Mrs. Mack called law enforcement after Jaheim broke the bedroom closet door and other personal property in the bedroom.
Staff intervened and shut chis ' door informing him to lock his door. CCIB Intake received a SOC 341 from reporting party Al Vanderhoeven who stated on 3/5/17 resident Paige Katler was arguing with resident
CCIB Intake received a cross report call from Marisa Romero with the County of San Diego CPS. Marisa stated she met with Detective Elizabeth Hundley to initiate the investigation of Gabriel Munoz DOB: 4/6/15 head injury. Marisa stated she and the Detective have statements from teachers regarding the incident. The Director was also interviewed and denied the incident happened. The Director continued to deny the incident even when the teacher Monika Huynh admitted the child hit his head when she fell.
She also informed her supervisor regarding the behavior seeking his help for the conduct to cease. Sgt. Bozeman
CCIB Intake received a call from Co-Complainant Adrienne Williams (323) 802-5315 who disclosed that the son of resident Eleanor Lanzarotta in room 204 punched resident David Lester in the face. Notably David is described as having dementia with aggressive and combative tendencies. Today, April 24, 2017, David entered Eleanor 's room and struck her which resulted in her son striking him back. The complainant was not aware if David sustained an injury from the encounter due to it being the end of the shift. The complainant reported David is not appropriately placed in the facility and poses a significant risk to the other residents in care.
CCIB Intake received an anonymous complaint from a parent stating a teacher named Pam Morgan hit their son Yared Ortega on the head. The incident occurred last week. The complaint was on 9/21/16. According to the complainant the child only speaks Spanish and reported to his parents that Pam hit him. The child is currently afraid of Pam due to her physical behavior towards him.
CCIB received a call from Shauna. Her son Kai Crupt (DOB 10/05/11) attended this day care. Shauna reported that this summer her son came home and told his parents that teacher Yolanda pulled his hair. They thought maybe it was an accident. A few weeks again Shauna got off work early and picked up her son (Around 1:30 PM).
On 02/21/2017, I, Officer Rivera Ayala, responded to the lobby of the Bethalto Police Department in reference to a Domestic Trouble. I met with the complainant, identified as Elaine L. Davison (DOB 07/13/1984). Davison stated that when she got back to her house from running errands, Davison found that somebody had “bleached and messed” her room. Davison stated that she was not sure of who would have done something like this. Davison wanted to have a report on file and for the Police to take pictures of what had happened.
CCIB received a telephone call with an allegation against the facility. RP called to report that a program participant of the Resource Center Candice McGurre (435-899-9390) called to stated that her children that attends this daycare told her that Ruth has slapped children on the hand and also shoved a child against the wall. RP did not have any more details of child(ren)names or when these incidents occurred.
The reporting party (RP) stated the resident 83 year old Iraj Ajir was found in his closet by his daughter Katra Ajir. According to the daughter he was covered in urine. The resident was also observed to have bruising to his arm and a burn on his leg. CCIB LPA McGaskey left message family representative Katra Ajir at (310) 498-2884 regarding which arm and leg was injured. According to the RP the resident 's room was covered in feces and urine.
On 11/5/16 at approximately 1554 hrs, I was dispatched to a fight at Davis Field (6050 78th Ave N, Pinellas Park FL). Upon my arrival I made contact with Yakail Brunson who advised herself and her sister was battery by a group of individuals at the game. Brunson was crying and was in distraught at the moment, I told her to relax and explain the details. Brunson advised she was jumped by multiple subjects in the crowd because she asked the Lakewood cheerleader coach why niece daughter wasn’t cheering.
The reporting party (RP) resident Lucia Fields age 35 DOB: 6/25/81, stated she was admitted to the facility on 3/17/16 due to a vehicle accident which left her with 2 fractures in her back and 4 fractures in her pelvis. The injuries left the resident unable to walk and requiring assistance with her daily living activities. The resident stated she was left in her soiled be and had to administered her own medication. The resident requires a different assisted living facility which will provide the level of care necessary to meet the resident 's

Overall, the simulation case of Mr. Ahmed was a positive experience that makes me feel in control and challenged by the situation. I perform multiple nursing skills and acknowledge acquired during lectures. For example, in reference to fluid, electrolyte, and acid-base balance information, I was able to identify the needs of a dehydrated patient.
Yesterday - July 25th, 2016 at 17:41 while I was getting ready to come to the compound, I received a phone call for the Guard named (Hugairah Ashour Hugairah) asking to change his days offs from Fridays and Saturdays to Sundays and Mondays for some family concerns. I wasn’t clear what was the exact reason since this is something belong to shift supervisor. I came to the compound and talked to his acting supervisor Alali, Ahmed about his case, and then we went together to see Hugairah at CAC1’s Sally port. He start talking about how bad his supervisor Rasoul is treating him, and how he asked him to change his day offs to Sundays and Mondays because he booked an appointment for his father to have a medical surgery and he needs to be with him, but his supervisor is not approving his request.
We, the employees of Harbor View Assisted Living wanted to inform you of misbehavior of one of the nurses. Mrs. Alicia Williams. She shows, unprofessionalism and improper behavior towards employees of harbor view and harbor care. Roughly speaking, Mrs. Williams raises her voice (yells), refuses to help when aid asks for help, and finally insults. Please pay attention to these unacceptable behaviors and for that fact take an action.
